It would be awesome for a notification to be sent out when a patient fills out the wellness forms so we can enter that information as promptly as possible.

Can we change the last question on the wellness form that reads, "Have you traveled in the past 14 days to any regions affected by COVID-19? (as relevant to your location)?
This question is very misleading....my suggestion would read....
"Have you or anyone in your household travelled/stayed anywhere outside of your usual home in the last 14 days?"

Hi, I understand that there is no way to change the wellness forms but it is really important to ask if the patient has a temperature greater than 100.4 deg.

I have a suggested question to add to the Wellness Form: Have you previously tested positive for Covid-19? If yes, then an additional box to answer date of test/diagnosis would be nice. The form now only inquires if the patient has come in contact with others who have been positive for Covid-19. Thank you for your consideration.
There should be an option to add a message to the top of the Wellness form. We were trying to avoid long or multiple text messages. For example, being able to tell people about our office fee that applies only during this pandemic time would make more sense on the form than in a text.

It would be a great idea to have the downloaded wellness forms to have the date they were submitted on them as well. Also, more suggestion categories because this doesn't really fall into a specific category that's listed.
To download wellness forms to Open Dental. You need to go to your site download the form into the download file on the computer. You then need to go into Open Dental, click on the patient, go into images and then import the wellness form. Not very streamlined at all. To many steps and to time consuming.

Is there a way that it can alert us on the dashboard when we receive a new completed wellness form so that it can be then looked at downloaded and saved in patients accounts. Instead of having to go to the admin portal and look for them,
We would like to be able to delete the wellness forms after they are completed and downloaded.
Hello, so only suggestion is that we are able to customize the wellness form. Given the recent update of the impact the new strand of Covid is making in the UK and how the symptoms are different, this would be very nice to have.
Hi I was wondering if you guys will update your wellness form and if so when that will be. Since patients are now getting tested for covid it would probably be a good question to add to the wellness form. Thanks.

Can Weave wellness form responses show up in the patient's information. Similarly to how patients confirm appointments.
We are current customers and have been using the Wellness Forms since they became available. As a suggestion and request, we want to ask if the questions "Have you tested positive for Covid-19" and when? can be added to the form.
We have had several patients mention to us that they haven't been around other diagnosed with the virus but they themselves have had it and the question is not asked in the questionnaire.

We would like to be able to update and make changes to our Wellness forms. CDC are constantly changing standard of care and we should be able to change the wellness forms . Thank you
We'd love to be able to edit the Wellness Form. Especially as COVID-19 is worse right now. We would really like to add a question or two.

We would love to have a notification if a patient completes the Wellness form and is symptomatic as well as a notification if someone has yet to complete the Wellness form. This would make our job much simpler. Thank you!

Wellness Forms Process Enhancement Request: When Patient's get their reminder texts and respond to confirm - on Dolphin Management appointment for that day a little green "C" appears for confirmed appointment. What I am wondering is if when they submit their wellness form for their appointment can we have Dolphin add a (green) "W". That way we can see right on the front of their appointment that they did their form per our new Covid19 office policies. We could even make it better by changing color to (red) and adding "!" after "W" for symptomatic alert ex: traveled.
This would highly improve the ease of process and time consumption in our day constantly having to look patients up on the Weave Admin Portal (because we see 40-50 patients per day and require each one to fill out the form).

Public Health in Ontario requires you to self isolate for 14 day regardless of a positive or negative test result. Please add these questions to the wellness form.
1) In the last 14 days, has a public health unit identified them/you as a close contact of someone who currently has COVID-19?
2) Has a doctor, health care provider, or public health unit told them/you that they should currently be isolating (staying at home)?
3) Have they/you been tested for COVID-19 in the last 14 days?
