Upload & Sync Patient Photos in Forms
Adding the upload photo feature to forms for new patients and have it integrate in Open dental. Nice feature to get to know your patients in advance.

Send a copy of the signed forms to the patient or guardian
After a patient signs a consent form, the Weave system would automatically email the patient/guardian a copy of the consent form they just signed.

Autofill e-signature
Our new patient registration forms are a lot. Is there any possible way to "autofill" date and signature? Similar to Docusign. It would save time and patience for many people

As of today, Forms automatically updates the date and time of submission in the PDF right below the e-signature. You need not add additional date fields in your forms to capture the date in the generated PDFs.
The functionality to carry over the e-signature from one from to the other in a packet is already on our roadmap. We will share an update as soon as it's released. Our team is working hard to bring many useful features and improvements like this in Forms. Thanks for your patience!

Form links on websites
It would be awesome if we could add a New Patient Forms link to our webpage. We've noticed that often people will fill them out even before calling to make an appointment!

Pull up patient's complete medical history so they can refer back to for updates
Requesting a way to attach patient's last or previous medical history to an update form to sign) Many patients do not remember or recall what they filled out last time. They always ask.."did I tell you X last time?, did I note X on my medical history last time?" It would be helpful to attach their medical history so they can see what updates need to be made
