Customize the text for Weave Forms message
I would like to be able to customize the text message that is being sent with the Weave Forms. Currently I copy and paste what I would like it to say each time, as you can imagine this is a but redundant.

Need an N/A Option for Required Text Entry Elements
There should be a built-in optional element to allow patients to bypass a required question if it's not appliable to them.
For example, if we have a short text element asking for "Subscriber ID" and the patient is Self Pay. If we make it required, they can't submit the form. But if we don't make it required, other patients can/will just leave it blank. We're having to type out instructions for half the questions, which just makes the form longer and more annoying for the patient to complete.

Ability to Group or Sort Forms by Category or Likeness
Please add the ability to label/categorize created forms or sort them in some way. Every time we need to send a form, we have to search through a long list of unsorted forms to find the one we need.
May also be helpful to limit the forms available by department or user.

Individual PDFs in Dentrix Doc Center
Hello. When we have a new patient complete our new patient pack of forms, we'd like there to be a better way for this to sync with Dentrix. Currently all the forms are in one pdf file in document center and it's not a very user friendly way of doing this. Each form should be put in separately so someone can easily access the medical history or dental history for example. Right now with all forms being in one large pdf file it is not easy to just look up a patient's medical history. Also the medical conditions allergies or…

Forms
Would love to be able to edit the forms once they are written back to dentrix. Patients usually forget some information and it would be helpful to be able to edit the form to fix that.

Improve Kiosk Mode
For OpenDental offices, it would be helpful to be able to (optionally) provide the OpenDental Patient Number as the key for Existing Patients, rather than First & Last Name, DOB and Mobile phone number. As an aside comment for consideration - not every patient has a mobile number (most do, so it not that big of an issue).

Digital forms
I am trying to make a online form for online referrals to eliminate paper referrals, time mailing/scanning, and multiple emails. My idea was to make a custom form but my only downfall was the option to upload xrays. They use that same tech for the Insurance card photo (Front & Back). My work around for the time being was to use said element for card and name it xrays even though it says picture of front and picture of back.
If there is anyway to make an area to upload photos beside insurance cards, would anyone else use it?

Multiple family member forms
We are pedo office using OpenDental. It is extremely frustrating to parents that have multiple children to fill out the "Verify Identity" portion for each child. There should be an option to verify a family once and then be able to choose what family members are being seen.

Medical Conditions Alphabetized in PDF
May we PLEASE have the rows of choices go vertically first rather than horizontally in a multiple choice question? Our office has our own Doctor-Specific Medical Conditions list for our paperwork. Writeback for 'click box elements' on forms
I just learned (after 2 months) that any click boxes on our forms do NOT get a writeback into Dentrix, they are left blank. Which makes it to where now my staff has to not only go in & edit the Response in the patients Questionnaire info but look at the Doc Center to ensure it is now correct. :-(

Transfer of Documents for patients to sign
We've recently switched from Yapi to Weave. One of the features we loved about Yapi was the ability to transfer documents to specific patients from Dentrix so that they could be signed electronically. For example, when we do a treatment estimate. We would transfer the documents to Yapi for electronic signature to minimize paper usage since we are a paperless office. Once they signed the document it would go into the document center automatically.

Please allow the option to integrate a packet that is sent to a patient to be separate pdfs once loaded into OpenDental. It is very time consuming to send these separate forms to patients, especially when they are a new patient and have at least 5-10 forms to sign! Everyone that has gone to a doctors office knows that there are at least a few forms to sign.
Or better yet, please allow the option to place each PDF form that is completed into different categories of OpenDental AUTOMATICALLY. It would be so convenient for offices who do not want…

Teeth Chart
It'll be great to have a set of checkboxes representing the teeth numbers. 1-16 on top and 17-32 on the bottom. That way when we're doing a consent form for extractions. Our assistants can select the teeth #s that we are working on. Also have the teeth #s in the order of how we think of teeth. So 1-16 on top line, and 32-17 on lower line.

It would be very helpful if a line was added for a phone number for the patient's insurance carrier (s) or at least made an option to add/edit.

Regarding digitation of forms, if each location used the same forms for the same company, it would be nice if Weave would provide those forms to each location and not ask the customer to have to clone and create for each location.

Security Measures
So there is a new security measure that makes the patient get a verification code prior to logging onto forms. SINCE then....patients have been coming to us in a panic saying that the SSL states the sit isn't secure. It doesn't matter if it is chrome, safari, edge, etc. Android vs apple, wifi or LTE, it doesn't matter. Please help us!!!!

Please create an option to choose which folder in the PMS forms are saved to/in.

"No To All"
Is it possible to have a "no to all" button? We have about 20-30 listed medical conditions on our registration forms. Clicking "no" so many times is tedious and time consuming. Patients are stopping midway through. It'd be great to have a "No to All" button for listed medical conditions.

Digital Forms for Existing Patients
We use Officemate and it would be nice when we send an existing patient packet, to have the patient address and phone number auto-populate. Right now it only populates the name and DOB. Want to minimize the amount of work for the patient while verifying everything is still up to date.
