  1. Dentist signing forms

    If it requires the dentists signature, why do I have to touch the patients mobile device to do so? Why are they not able to sign then send then I sign and approve? I am really unhappy with these forms and am starting to feel like I should have just stuck with Yapi.

    Hi Dr. Maria, If you have setup provider review in Forms, users with Forms Manager or Provider role can review and eSign submitted forms anytime anywhere as per their convenience.

  2. Show which user printed out the patient forms

    It would be helpful for a large practice if it would show which user printed off the patient forms. For tracking purposes.

  3. Form links on websites

    It would be awesome if we could add a New Patient Forms link to our webpage. We've noticed that often people will fill them out even before calling to make an appointment!

  4. Forms in Bulk Messaging

    If there was a way to send forms in bulk messages, this would increase our productivity. We want to be able to send out ins update forms in the beginning of each year.

  5. Treatment Plans for Patients to sign

    Treatment plans that integrate with the Sunbit option to pay. Having the option to create treatment plans at all would be nice.

  6. Add Forms to Mobile App

    Which Feature does this pertain to?

    What pain points/suggestions does the office have?
    They would love if Forms was on the Mobile App. Is that something that's coming in the near future?
    Also, they would love if they're able to select a provider on one of their forms before sending it to the patient.

  8. Integrate with Dentrix medical alerts!

    Integrated with Dentrix medical alerts so that existing medical alerts prepopulate in the Weave Form - and any updates the patient makes are then written back into the Dentrix medical alerts fields.

  9. PCI DSS compliance to enable secure capture of CC info in Forms

    PCI compliance so that you can capture CC information on a patient's form for secure storage in PMS. Competitors are doing this and I would love to see Weave get this done soon!

  10. Can we please make a Ipad app where patients can sign the forms?

    With Mconsent patient signed forms on the Ipad and they uploaded to the patient chart as soon as they were done signing can this be done?

    Have your tried using the Kiosk Mode in Digital Forms?

    1)
    You could login to the Forms portal on your iPad and ask your patient to fill out the forms or 2) ask them to scan the QR code to fill it out on their personal mobile device.

    Depending on your writeback settings, Forms submitted via Kiosk mode will automatically sync to your PMS. It will also attach the PDF to your patient's record in your PMS.

  11. Designating Document Groups to Use Intellicare (in Eaglesoft with Weave Forms)

    We use Weave and Eaglesoft. When patients complete a Form through Weave, it imports into our SmartDocs in Eaglesoft in the form of a PDF and the PDF gets placed into a "Patient Identification" document group category.

    We would like to use Intellicare(Eaglesoft feature) to let us know when patients need to update their health history(for example).
    In order to use Intellicare for this purpose, we would need our Health History Forms to go into a specific document group and other forms to go into a separate form (instead of ALL forms going into the "Patient Identification" group.

  12. NP Form Packets

    Having the ability to send out multiple forms in one text is great. However when we get them all back there are now just one file and we cannot piece out individual forms to place them in different areas of Dentrix. It would be nice if they could still be individual forms. I don't want to send 6 different texts just so I can keep the forms separate.

  13. digital forms patient name changes

    When patients fill out and submit digital forms, however they fill out their name on the forms automatically changes their name from our software. For example, patient's name in the our software is John Doe. but if patient types in Joseph Smith, then it will automatically erase John Doe from our software and puts in Joseph Smith, thus no longer we are able to locate the patient in our software. I understand that patients may make mistake when typing in their names or they may put in their nickname instead of full name, thus may create an error. I request…
