  1. Kiosk mode

    Select multiple consents at one time for one patient. Instead of doing one form at a time and entering all the patients information every time.

  2. Multiple document selection in kiosk

    Be able to select multiple single documents in kiosk mode for the patient to complete that are not made into packets. When multiple documents need to be completed, Finding the patient, choosing one document, then having to do this again is very time consuming. Especially when the patient is standing at the font desk. We should be able to search for the patient once, then select multiple documents for them to complete in one selection

    Hi Susan, Thanks for your feedback. Here's the recommended solution: Please create one or more packets with the most frequently used combinations of forms so that they can be filled out in one selection. Please try this out and let us know if this solution works. Thank you!

  3. Patient ID in kiosk mode- search by ID

    Make finding the patient faster in kiosk mode. Entering name, birthday and phone number for each document is time consuming. Search by patient ID should be available.

  4. Red "Yes" lines on Medical History

    On the medical history form, when the patient clicks "yes" on something can the line turn red like it did in Mconsent? This flags the doctor when they are reviewing the form.

  5. Designating Document Groups to Use Intellicare (in Eaglesoft with Weave Forms)

    We use Weave and Eaglesoft. When patients complete a Form through Weave, it imports into our SmartDocs in Eaglesoft in the form of a PDF and the PDF gets placed into a "Patient Identification" document group category.

    We would like to use Intellicare(Eaglesoft feature) to let us know when patients need to update their health history(for example).
    In order to use Intellicare for this purpose, we would need our Health History Forms to go into a specific document group and other forms to go into a separate form (instead of ALL forms going into the "Patient Identification" group.

  6. Improve Digital Forms Kiosk Experience

    The New Form Platform on Weave is not functioning. Not only you guys launched in the middle of the day without notifying your customers, it is not even working properly. This caused a major inconvenience in our office and we looked completely incapable in front of our patients. The purpose to using the forms dashboard is to go completely digital and with this launch, we ended up printing out tons of forms because the Kiosk wasn't verifying the patient information even when everything was typed EXACTLY as is. We have 73 patients tomorrow and we are now staying after hours…

  7. NP Form Packets

    Having the ability to send out multiple forms in one text is great. However when we get them all back there are now just one file and we cannot piece out individual forms to place them in different areas of Dentrix. It would be nice if they could still be individual forms. I don't want to send 6 different texts just so I can keep the forms separate.

    Some besides Admin needs to be able to access forms. Maybe the Advanced team member.

  10. Have the office name & Contact info/logo on top of registation form.

    Have the office name & Contact info/logo on top of registration form and on the top of the medical history form. That way if we need to print off for any reason, our office info is on the forms.

  11. Remove Eaglesoft Medical History form

    PLEASE provide the option to inactivate or delete the Eaglesoft Medical History form! We are an Eaglesoft office that does not use the Medical History form - we are pediatrics so we ask lots of additional specific questions about bottlefeeding, thumbsucking, etc on a separate Medical Update form.

    We have a QR code set up for Kiosk mode for parents to scan and sign the forms, but the ES Medical History form is always top of the list, and it confuses the parents who occasionally complete the wrong form even with direction. Please provide a way for us to remove…

  12. Inactive Patient accounts Filtered out or ID field as the first piece of information entered

    Consent forms on the ipad - we are not being able to pull up the existing patient due to the fact that our system has many duplicate patients with one of the charts being INACTIVE. The problem lies in that the inactive patient chart still has the exact same data that is in the active chart for Name, DOB, & phone number. I have talked to eaglesoft and they cannot mass delete inactive charts phone numbers. The only solution I can think of is if Weave added the Eaglesoft Patient ID field that Mconsent had. We never had this problem…

  14. Pull up patient's complete medical history so they can refer back to for updates

    Requesting a way to attach patient's last or previous medical history to an update form to sign) Many patients do not remember or recall what they filled out last time. They always ask.."did I tell you X last time?, did I note X on my medical history last time?" It would be helpful to attach their medical history so they can see what updates need to be made

  15. Forms

    For OpenDental offices, it would be helpful to be able to (optionally) provide the OpenDental Patient Number as the key for Existing Patients, rather than First & Last Name, DOB and Mobile phone number. As an aside comment for consideration - not every patient has a mobile number (most do, so it not that big of an issue).

  16. Text Popup for unentered fields

    When patients fill out digital forms (health history forms for example), they are unable to proceed if they do not enter something like N/A or None for fields they do not have. This primarily is an issue with insurance information, not every patient has health insurance information. May a feature or text pop up stating "If none, enter N/A" so that they may continue submitting the remainder of the form be implemented? Thank you!

  17. Fax sorted by fax dialed, not incoming fax number

    Since weave doesn't sort anything by department (yet another idea), we have 15 fax lines coming into one practice. Incoming faxes are listed by "incoming fax number". That doesn't help identify what department/fax number was receiving the fax. We get over 100 faxes per day. It would be nice to identify what department was supposed to receive it - either by listing the fax number, it was sent to, or allowing user to change the "name" of the fax number.

  18. Verification

    When beginning a form and putting patients information. It will be more time efficient to just put patients chart ID and not put patients full name, date of birth and phone number.

  19. Doctor review and approval feature

    There should be a feature/setting that allows you to select a particular form (such as medical history) for doctor review. When patient complete form it would enter a queue for the doctor to review and approve later. Also, why isn't the forms icon/option available on the mobile app? Development of digital forms feature seems rushed.

    I want to be able to delete a from from needs approval if I choose not to use it or don't want it uploaded.

    If we send a form to a parent and they fill out the data for their child it appears under the parents name so if we sync it won't go to the child it will change all of the parents info.

    Also there should we an alert to any mismatched information so that it we can compare and update before syncing.
