Customize Templates for Forms Via Email
Be able to customize the message that's sent for Forms via email! Especially for new patients so that their appointment date and time can be added.

Doctor review and approval feature
There should be a feature/setting that allows you to select a particular form (such as medical history) for doctor review. When patient complete form it would enter a queue for the doctor to review and approve later. Also, why isn't the forms icon/option available on the mobile app? Development of digital forms feature seems rushed.

Select which froms for Manual review and write back
When we switched from mConsent to Weave forms we lost the ability to have the provider electronically sign the from in their own portal; if weave would allow an option in the Form Editor for the office to select which forms could be manually reviewed the provider could then electronically sign it before it is written back.
Post COVID I am surprised that we have the ability to have it on the patient phone yet ask the provider to touch the patient phone to electronically sign.
Right now if we make all forms manual we loose the benefit of which…

Designating Document Groups to Use Intellicare (in Eaglesoft with Weave Forms)
We use Weave and Eaglesoft. When patients complete a Form through Weave, it imports into our SmartDocs in Eaglesoft in the form of a PDF and the PDF gets placed into a "Patient Identification" document group category.
We would like to use Intellicare(Eaglesoft feature) to let us know when patients need to update their health history(for example).
In order to use Intellicare for this purpose, we would need our Health History Forms to go into a specific document group and other forms to go into a separate form (instead of ALL forms going into the "Patient Identification" group.I have…

I would like to see Weave Forms integrate with Compulink. In The winter of 2021, I took a forms demo and at that time was told that Forms integrations was scheduled for June 2021. When will this be available? I am starting to demo other products that are already integrated.....

Improve the layout of forms created in Form Editor
As far as the forms go, it would be great if the editor had control of the spacing as the documentation is very choppy and spacey. Also, if you can find a way that customers can logo their forms in specific places not just centered at the top of the document in font 48 size. A way to make them prettier and more eye catching would be ideal. Thank you.

Health/Medical History: 1 Question With Boxes to Check/Uncheck Instead of Separate Questions For Each Condition
It would be much easier (less tedious) for the patient, on the health history part of a digital form, if instead of individually asking 100 questions about their medical conditions that they have to click yes/no to, to just have one question with all the boxes of medical conditions that they can select all that apply. So they can either just check 1 box that says None of the Above, or just click the couple of boxes that correspond to the medical conditions they have, instead of having to tediously click yes/no to 100 different medical condition questions.

I am writing to express our need for a patient satisfaction survey feature within Weave that does not require going through the HIPAA-compliant "Pre-Admission Registration Form." This survey would contain information solely about the experiences of the visit at our facility and with our providers, without including any patient-identifying information.
As you know, obtaining patient feedback is crucial for assessing and improving our quality of care, in addition to the reviews. In addition, we need a custom patient satisfaction survey that allows us to export data reports in a format that suits our needs (e.g., weekly, monthly).

Hide information in completed form
It would be useful to be able to hide information from the completed form. For example, we have a price list section on the form, which is information that is useful for the client, but not for office use. Being able to hide the unneccesary info would speed up workflow, and help condense the forms.

Form builder - cut and paste option
It would be handy in updating/reformatting digital forms to be able to CUT & paste elements rather than copy/pasting only. For example, all the forms that Weave digitized for us were single-section forms, making them very long. I discovered the Section tool and like the usability of that. However, in order to update the forms to use Sections, I have to drag each element down into the new Section, which is pretty time consuming and annoying, especially when the Section doesn't "grab" the element you've just dragged all the way down the page and the element stays put in its…

Clone from one form to another
Option to clone sections/fields from one form to another, rather than only within the same form. For example, we need health history info on multiple forms and it would be nice to copy/paste that section across forms since it's a checklist and can't be copied/pasted from a Word or Google Doc.

We asked specifically of our sales Weave Rep if Weave had an App for the Kiosk I Pad and were told yes. Unfortunately this is not true. There is no App. An App would make checking in patients much more stream lined and easier to check in patients for forms and new patient information.

Multiple choice conditions unselecting previous answer
When creating a condition within a multiple choice, selecting the option that is related to the condition opens the second multiple choice section as expected, but it then deselects the previous answer.
For example:
Multiple choice with A, B, and C.
When C is selected, another multiple choice option appears, (condition) options D or E. This is what we want.
When D or E is selected, C is deselected. For record keeping's sake, we need Weave Forms to indicate C was selected, as well as D/E.

autosignature
You should have a place where we can paste autosignature for regular staff. This why the staff does not have to sign the form each time we get it. We can pick from one of the 10 staff and paste the signature to the box.

Confusing verbiage on sent forms
In the app, under sent the name of the packet that will be sent out is the the actual packet. They are all listed as "office visit Packet" which is the name of a different packet. Once it queues up the patients to send the reminder to with a form it then reads the filters to send the correct packet.

7MB image size for uploads too small for todays smartphones. Uploading pictures of drivers licenses or insurance cards is virtually unusable for anyone over the age of 40 that has bought a phone in the last decade.

Uploading forms
Is there a way to pre-load forms with scheduled paitents instead of loading each one at check in? For example, we see about 35 patients per day. It would be ideal to upload all the forms/packets to each patient and "save" them. This way, when patients checked in, we could simply select the patient and hand over the tablet for completing the check in process. This would greatly improve office efficiency.

Filtering Auto-Messages to know which patients to send what forms to while using Dentrix
We use Dentrix and there is no way to filter out which patients have submitted their "New Patient Packets" (all were new when we started with Weave a couple of months ago), and which have not. The filter options do not make sense with Dentrix. We need to use the "Status" for when we reach out to confirm an appointment or when the patient is here, etc. The "Type" is basically the description we use for the appointment itself and changes with each patient and appointment. There is literally NO WAY to filter it out.
