Autofill Names in forms
Since you have to authenticate a patient with their name, phone number and birthday. It would be great that those fields be auto-filled in the forms, so the patient does not have to keep entering them on each form. I have made a packed for my new patients and each form in the packet requires name and date of birth to be entered manually. It would be great if these field would be filled automatically right after the authentication.

Hi Sam, If you have set up forms to be sent automatically (via auto-reminders) or manually from the desktop app, then the following fields will be auto-filled and locked when your patient fills out the form: First name, last name, DoB and phone number. The phone number can be edited after confirmation.
Moreover, the established patient doesn’t have to fill out the pre-admission registration form in this case because the form link is uniquely generated for them.
The only condition is that these four fields should be primary fields; not element fields.

Forms for multiple or family. include patient name on the links so they know who they are filling the for out for. We have had two issues with this so far. the mom clicked one of the 5 links that were sent to her and put her sons information in on her link. it was a mini disaster trying to sort all that out. we were only able to do so in comparing xrays to get the charts corrected. this also happened to two new patients. also, if the forms could be put into the patients smart doc automatically that would be awesome! currently, recall max is able to do that.
