History Form should NOT be able to change a NAME or DOB when a family member fills it out for the wrong person.
Currently we have had a few family members that have the same phone number on file fill out a history form for a different family member.
The message will indicate "Hello Jill, please fill out this form" or something to the like. Then a family member will fill out the form in THEIR name instead the intended recipient. So Suddenly instead of having Jill Smith in the system with her DOB, we have 2 accounts with her husband's name and DOB.
A NAME OR DATE OF BIRTH CHANGE SHOULD NEVER BE AUTOMATED. This should flag the system for our review. This has happened multiple times to us. Not every family can have an individual phone number for each member, especially with children.
