Form notifications
It would be great to get a notification when a Form has been completed.

Thank you for creating the wellness forms!
Is there anyway you can come up with a way to have the ability to send these forms individually to patients instead of group format? also, can you combine the curbside instructions and wellness form together? thank you, for your help, Ruthie:) -

Our office would like to edit the wellness form. There are additional questions TDA requires for patient pre-appointment screening.

On the Wellness Form related to COVID-19 - We would like to also include the following question(s): Have you traveled out of the country in the last 14 days or have you been in contact with anyone that has? Have you come in contact with anyone that has tested positive for COVID-19?

Please update the wellness form as it is out of date an not usable:
Does patient have fever or have you/they felt hot or feverish recently
(the last 14 days)? Yes No
Does the patient have shortness of breath or other difficulties breathing? Yes No
Does the patient currently have a cough? Yes No
Does the patient have any other flu-like symptoms, such as gastrointestinal upset,
headache or fatigue? Yes No
Has the patient experienced recent loss of taste or smell? Yes No
Has the patient been in contact with any confirmed COVID-19 positive patients?
Patients who are well but who have a sick family member at home with COVID-19
should consider postponing elective treatment.

Post Screening Form after visit to see if they are now experiencing symptoms 14 days after visit at the office. By Law we are required to track this so can this be made please

Can we update the wellness form to ask if someone has traveled out of state, and give them an option to choose what state they visited in the past 2 weeks?

You guys need to update your wellness form because you are missing FOUR questions that recommended by the ADA that we should be asking patients
.
You are missing the following questions:(1) Any other flu-like symptoms, such as gastrointestinal upset, headache or fatigue?
(2) Have you/they experienced recent loss of taste of smell?
(3) is your/their age over 60?
(4) Do you/they have heart disease, lung disease, kidney disease, diabetes or any auto-immune disorder? -

Please use wellness form (questions) from ADA toolkit, this is most current, otherwise we cannot use this feature. Thank you!!

On the screen for the curbside waiting room, it needs to update when we add patients to the schedule.

On the wellness form if would be very helpful after you print the form to have it marked with a P or something so that just by looking at the screen you know which have been done and which have not. Thank you

We are in need to making edits to the WELLNESS FORM and the ability to add questions.
Do you have a cough?
Do you have a fever now or have you had fever in the last 14 days?
Are you experiencing shortness of breath?
Have you traveled outside the state in the last two weeks?
Have you come in contact with someone experiencing symptoms of COVIC-19 in the last 14 days?
Have you had COVID-19 and symptom free for 21 days? -

Our office thinks it would be beneficial if we were able to edit the Covid screening questionnaire, especially in light of the newly added symptoms.

With the curbside waiting room for COVID-19 we really need a sound notification when patients text the office. We are short staffed and not always at our desks because we are with patients and sanitizing patient areas. Sometimes people are waiting longer because we don't see the pop up for a few minutes.

The wellness form has a great start, then drops off completely. With out looking into the admin portal and cross referencing each patient, I can not tell which patient has filled it out or not. Having a note or icon under the history section would be very useful."Pt filled out WF".
