Is there a way we can modify the wellness form so that it has the questions required by the american
dental association.

I recently got an update from the state that requires certain legal requirements for screening patient for COVID-19 before seeing the patient. I need you to update the COVID form that is being sent out to include the following questions:
1. Recent history (past 14 days) or currently have a fever
2. Recent history (past 14 days) or currently have a cough
3. Recent history (past 14 days) or currently have shortness of breath
4. Recent history (past 14 days) or currently have a sore throat
5. Recent history (past 14 days) or currently have muscle aches
6. Recent history (past 14 days) or currently have chills

Hi, we love the wellness form, however it would be nice for us to edit this form so we can use the ADA format that they constantly are updating. Just an idea!!

I like the wellness form. However, the ADA has just put out a new Patient screening form. Is there any way to copy it/some of those additional questions in the wellness you already have.

WE need to be able to edit the wellness forms
also once we get them back we need a way to delete them so we dont have a giant list of forms.

Hello-I wanted to reach out to help Weave and all the other dental offices using this system. The ADA has released a toolkit that includes a Patient Wellness Screening Form. I have appreciated the Weave form and have been using it, however, because this is a rapidly developing situation- it has become out of date. I am happy to provide you with a copy of the ADA form if you would like to use it as a reference to update the Weave Wellness Form. I have emailed it to Erik H. in Support (ticket #577273)..

Additional questions in Wellness form to include new CDC Covid-19 symptoms. These include: chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of smell or taste.

There needs to be an easier way to send just one patient the wellness and curbside check-in from the scheduling icon where you would send a group text. Having to go to the admin portal copy, paste, is a lot of steps. Please fix! Thanks!

We are requesting to add some questions on the Wellness form. Have you or anyone that lives with you been tested for the Covid 19 and a waiting or results.

Would it possible to make it possible to edit the COVID questionnaire to suit out TDA recommended questionairre.

Analytics - We would like to be able to download all the data and mine it. The numbers coming in now to the Analytics doesn't appear to be accurate and I struggle to tell how my people are performing to answer phones.

We would like to download the data from numerous wellness surveys into a CVS file so we can keep record and present it if requested. It takes too much time to download and save each survey individually.

It would be greatly appreciated if we could get a notification text when patients complete the wellness form. As an office we are going to be so busy with all the “new” norm of getting the office running it is going to be hard to keep check the portal to make sure each patient filled out the form. Thank you!

Is it possible for Weave to edit the COVID 19 screening form? It asks if patients have traveled outside the COUNTRY in the past 2 weeks. Since most people have not, we are more interested in patients that have traveled to/from other states such as NY or NJ or other regions experiencing a high rate of cases. Thank you

It would be great to have the ability to delete downloaded wellness forms or ones that have been read to eliminate clutter and make the ease of confirming submission among 50 patients a day . Thanks for all you are doing to support this challenging time.
