I've emailed a form my malpractice provider gave us for patients to Erik Houmand who hopefully forwarded it to you.  I'm also copying and pasting the Texas Dental Association's recommended questions for reopening.  If we could be able to edit the Wellness questionnaire for our practice or at least you all add the additional questions to make it more comprehensive, then this would be a great feature to use with patients.  Thanks!

Texas Dental Associations COVID-19 questionnaire:
1. Do you have any of the following respiratory symptoms? Fever, Sore Throat, Cough, Shortness of Breath?
2. Have you recently lost your sense of smell or taste?
3. Do you have any GI symptoms? Diarrhea? Nausea?
4. Even if you don’t currently have any of the above symptoms, have you experienced any of these symptoms in the last 14 days?
5. Have you been in contact with someone who has tested positive for COVID-19 in the last 14 days?
6. Have you traveled outside the United States by air or cruise ship in the past 14 days?
7. Have you traveled within the United States by air, bus or train within the past 14 days?

The Curbside options need to be more user friendly. We have a lot of emergency patient that come into our practice and I need to be able to send them a curbside text and wellness forms individually. This can become a nightmare if we are not able to use it quickly and efficiently.

It would be great if you could turn OFF the text feature that says Reply STOP to unsubscribe at the bottom of every text. There are many times I have a complete conversation with a patient through text and it gets repetitive.

We would like to customize the "Wellness Form" for COVID-19 screening.

Wellness form:
1) Allow for us to modify the questions
2) The pandemic is no longer outside the US and needs to change from 'outside the country" to 'outside the region'.

Wellness form customizable and on the wellness form page on the portal can we have the page indicate pending and received...so that we can see who we sent the wellness form to.

Hi, I wanted to see if it was possible to customize the questions on the wellness form. I think different questions are more pertinent to certain professions, geographical area, etc? Also, question created a month ago might not be relevant when we go back to work in several weeks. Thanks!

Patient Check In platform.
Is there a way to add a portal or other means to create a patient check in platform? This way patients can check in and update their information while in their cars and also sign important documents that we can attach.

Hello! In the Wellness Form section it would be super useful to add date/time to when forms are submitted, and being able to sort by chronological order. Thank you!

I love the covid 19 wellness form however it is lacking a notification when the form has been completed by the patient. It would be great if weave would add a notification when the completed for has been uploaded so we can review it.

Can there be a time and date stamp on the wellness exams please. Its difficult to know when someone has filled out the form.

Add addition custom/edit wellness forms

On the new Wellness forms, it would be nice to be able to delete a patients response after we have seen them. Otherwise my list is going to be miles long. Thanks

I think the Wellness Form that Weave created is fantastic. Can there be a way to add a timestamp to each form filled out? Also, can they add a virtual signature to the form? Thanks!

please can we have an ability to see and export the wellness form so we can export as pdf and save it so it can be save to patient chart. asap please
