We have had patients that do not have e-mail have a difficult time submitting their wellness form to us. Is it possible to remove that field from the link?

Add the Date Submitted to the Patient Wellness Forms. I assume most practices, like ours, are going to want to download the form and put it to the patient's chart. It would be nice if the form had the date submitted so that when we look in the chart we can see / are protected that we followed the proper COVID-19 protocols.

Add a button for covid wellness forms to be viewed without logging into the admin portal...this should be a button in the app/ on the software for all office staff to check.

Covid Screening form: Could you please add to the form: "if you have answered YES to any question, please explain" and make the explaination field mandatory.

When confirming appointments patients should not be able to confirm without completing the patient screening form attached to the appointment reminder they get 3 days before appointment

We would like to have questions removed from the Wellness form. The over 60, heart,lung,kidney, diabetes, auto immune, and traveled question. For our practice we already have this information and do not need to ask our patients this again. As far as travel that is irrelevant . If anything maybe the option to change these questions per office would be nice. Thank you

It would be very useful to list Appointment date with the returned Wellness Forms. I want to see who is scheduled for my current day has completed screening, and who I will have to screen in person or by phone.

There is a question on the COVID wellness form that is a bit confusing. It would be nice to be able to edit the questions. The question asking if they have been to an area affected by COVID. People aren't answering correctly and then show up for their appointment and have to be turned away. It would be better if it asked if they have traveled outside of Maine. I think that would get more of a response. Thank you!

I LOVE the wellness forms but I think it would be great if after it is completed to be reflected somewhere other than the admin portal. For example have a symbol or note populate on the appointments in the schedule on the weave app. Or possibly have it integrate back to the PMS. This would save a lot of time for busier offices or offices with limited staff.

it would be helpful if the wellness forms were archived once downloaded; or if once downloaded it changed from black type to gray ( or any other color). I realize the forms are dated, but if multiple staff members are working on keeping up with importing these forms into our PSW, this would be helpful

Being able to delete or mark as read a completed wellness form once it has been reviewed or moved to the patient's document center would be a big time saver. Right now there is no way to tell if a form has been handled and we have multiple people working our schedules so they are wasting time repeatedly checking to see if a form has been reviewed. Please add a delete feature or read feature to make the management of the wellness forms more efficient. We started to use this to save time but it isn't because of the inefficient management features. Thank you.

It would be great to be able to delete wellness forms after they are downloaded. At the very least, it would be nice to at least be able to check them off as you go. This would help with organization, especially since interruptions happen consistently in a busy office.

Can you please add a question asking if the patient has been confirmed positive within the last 14 days. And add a 2nd line asking if they have tested positive if they have had a confirmed negative test after. Thank yous

It would be nice if we could be notified on when a wellness form is completed, like the fax, or text notification.
