"preferred name/nickname"
Patients that have a "preferred name/nickname" would like to have the reminder text state that name.
This already exists! Just use the Preferred Name dynamic field - if there isn't a preferred name, then we'll fail over to the normal first name.

Matt I had a weird incident 2 weeks ago where I called the office to test the phone systems over a weekend and I got the following auto message back "Hey {preferred_name}, thank you for contacting the office of _________! You have reached us outside of our regular business hours, but we will get back to you as soon as we return."
It actually said "{preferred_name}" in the auto text message, yikes. The weirdest thing is I can't even find that template anywhere... was there some kinda upgrade on March 24th ~11am PST that might have triggered old alert templates to bug out?
