Choose which forms to display in Kiosk Mode
We'd love to be able to choose which forms are displayed when a patient selects either "New Patient" or "Existing Patient" in kiosk mode. Currently both options show all our forms which has the potential for confusion. Thank you!

Thank you for your patience and enthusiasm.
I'm excited to announce this improvement to our Kiosk Mode Patient Self Check-In, making patient check-ins smoother and more efficient. Within Kiosk Mode in the Digital Forms Dashboard, you can change how your forms and packets appear during the Patient Self Check-In flow. Pin frequently-used forms to the top, hide irrelevant ones, and apply filters to display only essential forms/packets to patients.
- While on the QR Code page within the Patient Self Check-In flow, click Customize Kiosk Forms in the top right corner.
- Select the Packets or Forms tab at the top.
- Use the Sort by dropdown to choose which order the forms/packets should appear for patients.
- Click the More icon next to a form/packet to Pin To The Top or Hide This From the Forms List.
- Click Save when you are finished.
- Select Confirm.
Hidden forms/packets will not appear for patients in Kiosk mode when selecting which to fill out, but will appear at the bottom of the list in the Customize Kiosk Forms page in the Digital Forms Dashboard. You can use the More icon to show the form or packet again.
We've also enabled the option to download the QR code image for Patient Self Check-In. That image can be easily printed and displayed or shared among staff for streamlined access.

When a patient takes a picture of the kiosk icon, we want to only show to update 3 forms (new patient registration, medical history update, or record release form. etc). Currently we have many forms on our page but would like to hide access to patients to make it easier for them to know which to update

When you click on the link or scan the QR codes. it shows everything to the patient. Can we get something to show only what we want to show the the patient and hide the rest. It gets very confusing to the patient when they see all different types of forms and packets. Also there should be another way to fill out multiple family members.

the main reason we hired Weave and spent the money to have this each month is to help eliminate staff and waste of time. if you want me to pick up the ipad, fill out the fields, pick which form to fill out, then hand it to the patient and wait for them then get the ipad back and click more buttons to submit, then what is the point??!! the purpose was to have a QR code for the patient to immediately fill out a form i need and populate my EHR. if we can't do this then i'm not sure why we have weave forms

Agreed 100%! Our New Patients would avoid selecting the forms that are not intended for them and would make our front desk support more productive.

The online forms are great. We would like to be able to edit the view in the Kiosk mode. Patients don't need to see all the forms, only the packets we created. If we can't edit what they can see, make it so we can edit which position the packets are in, so the most popular one is to the top

We would like to be able to rearrange how the packets and forms are. For example, for the new patient, having a new patient packet on the top would be better then having them see all the forms we have available. It will be less confusing for patients.

We have several forms uploaded for patients. I would like to be able to rearrange the most used ones or to even "hide" the ones not used but not delete them so we have them when needed but we dont have to scroll through all the forms that we dont use regularly

After qr code scan- log in, when electing either new patient or established patient assigning the packet to choice instead of allowing the patient to choose individual forms. i.e: new patient packet for new patients and update forms for established patients
