Eaglesoft Medical History Writebacks

Medical History answers are not integrating to the Eaglesoft Medical History Form, which in-turn creates a pop-up medical history alerts with in eagelsoft.

Customizable Medical History (Hx) form for Eaglesoft is LIVE! 🎉
Thank you so much for your trust and patience! You can now customize the questions and format of your Eaglesoft medical history form as per your practice's needs and easily import it into Digital Forms. Just follow these simple steps:
- Customize your medical history form in Eaglesoft
- Set it as Default
- Go to Form Builder in Digital Forms and click the Eaglesoft Medical History form
- Click the Sync button to import it into your Forms list
- Add it to a packet if required.
We have more such highly requested features and improvements lined up for release. We'll keep you posted as and when they're ready for roll out. Please take a moment to rate this update and continue to help us make this product better.

We are using eaglesoft and I have concerns with the Medical history.
All the conditions should show as it is in the eaglesoft medical history and smart doc.
For example: Under Medical condition for the question Do you have or had any of the following conditions, If patient says Yes to any condition it just show that one not all the one with NO answer. Eaglesoft medical history doesn't get signed but the one in smart doc gets signed by the patient.
Please update the medical history which gets saved in the smartdoc same as it is in the eaglesoft medical history folder.

PLEASE provide the option to inactivate or delete the Eaglesoft Medical History form! We are an Eaglesoft office that does not use the Medical History form - we are pediatrics so we ask lots of additional specific questions about bottlefeeding, thumbsucking, etc on a separate Medical Update form.
We have a QR code set up for Kiosk mode for parents to scan and sign the forms, but the ES Medical History form is always top of the list, and it confuses the parents who occasionally complete the wrong form even with direction. Please provide a way for us to remove this form from our form builder, or inactivate it, or even rename it so that the parents aren't confused. Thank you!

We are using the new Medical History created by Weave for Eaglesoft. When the pt selects "Do you any Medical Condition" and NO is selected, the med history that integrates in to Eaglesoft is not appearing properly. All of the NO boxes are not being checked under each listed Medical Condition instead they are left blank. Also It would be better to have the pt reviewed the list of Medical Conditions we have listed and then select NO TO ALL. Currently the pt is asked "Do you any Medical Condition" and if NO is selected the list is not provided for the pt to review. Some patients may not recognize the Medical conditions listed as a medical condition past or present. Also, the question should be worded the same as the medical History in Eaglesoft. "Do you have, or have you had, any of the following?" I would be happy to send a copy of that form.

I see a new form has been added to our list of documents this form does integrate but does not have the questions that our customize Eaglesoft Medical History form has. Can we please have our form edited to add these questions, they do not have to integrate but just be added to the form so we can see them once it synced we can view it in the document center?

Please create a way to default all answers to "NO" on a form, specifically our Medical History Form.
