We switched to Weave from NexHealth. NexHealth was able to automatically import medical pop ups from patient's medical histories that they filled out electronically. I just found out that Weave does not offer this so now we have to manually enter those in for every single patient that fills out forms. This takes a lot of time. PLEASE add this feature. I find it to be less helpful to have patients fill out their forms online if it does not offer cutting out the manual data entry all together.

This is a standard feature of other online form systems my personal medical providers use. I confirm the info that's in there vs filling it out each time I check in. Also critical for our practice, which is a pediatric office, and we see families with multiple kiddos. Saves parents time and effort if info can be entered once, then confirmed/changed in the future rather than filled from blank every time.

We would like for Weave forms to save the patient's information from previous forms, specially for the Medical History which is usually updated yearly, it would be nice if the form not only recognizes it is an existing patient and prefill the patient's name which it already does, but also saves and prefill any medical conditions on previous responses patient had the previous year, that way they can just make any changes if needed and sign the form, it would definitely improve the patient experience and speed up the process with medical histories, specially with older patient's that tent to have more conditions to fill.

We use Officemate and it would be nice when we send an existing patient packet, to have the patient address and phone number auto-populate. Right now it only populates the name and DOB. Want to minimize the amount of work for the patient while verifying everything is still up to date.

The office would like the ability to pull previously entered information into a new form that a patient is completing. They have had pushback on existing patients completing their medical or dental history forms annually because they have to enter in new data even if the information is the same. They would like the option to review and submit if the information is the same, or update if anything has changed.

"I am testing the forms feature in my office. We’re going live with them in a couple of days. I noticed when a patient sends back their form the only way to have their name printed on all pages is to have them type it multiple times. Just like in the hospital, the forms should have the patient’s name without them having to type it multiple times. The Name, DOB and whatever other information we have in the weave portal should auto-populate. By the time a patient comes to office, we have most of the information in the chart, the system should be able to bring that over from the form so the patients can verify and correct, if needed. The reason why I decided to incorporate your system to our current workflow is to make tasks easier for the staff as well as for the patients . If an electronic form needs to have the name input several times, it is as cumbersome as a paper form. I would think it should be easy to code as a header or footer on the imported PDF form."

Please get the patient information to uni-formally populate between all forms so patients do not have to keep redoing it!
