    Patient Forms - We are new to these and there are a few things we are struggling with in our office.
    1) The length of the forms: We submitted a 1 page paper for Health history updates. When this form is completed by patients and returned to us, it prints out as multiple pages. This just isn't economical. The layout of the forms needs to be more concise (the way they were turned in to you) in the way they are returned to the office. The other issue is that if the patient doesn't mark a box, the section is left blank, and we simply get no feedback. For purposes of allergies, and health problems it would probably be better if it was a yes or no question so the patient was required to answer. We have the same issue with our New patient Registration form.
    2)Not all the data transfers over to Dentrix - this is an issue. If we are asking for it on the form we need to see it.
    3) It was my understanding that New Patients information would automatically upload as well as existing patients. This does not seem to be the case. A smoother transfer of data for all patients would be great.
