As of right now, there is a LEGAL issue which has been created from me transferring over to weave. I currently cannot use any of my consent forms. There is no functionality for the doctor to edit a field (i.e. put the procedure being done) then having the patient sign. The suggestion that was provided was I have the patient sign and then sent to me for review/edit. This is ILLEGAL. I cannot edit fields once the patient has signed. This is with literally every and any contract in the United States. I already had some patients question me why they were signing consents when nothing was added in the “procedure” field. You can imagine the questions that you would have if you were a patient with a doctor who was asking you to do the same thing. I am sure you would have some issues/refuse to sign the document. Please, please, please….stress to the developers how important this is to fix. This is an issue of LEGALITY.

Patient Forms - We are new to these and there are a few things we are struggling with in our office.
1) The length of the forms: We submitted a 1 page paper for Health history updates. When this form is completed by patients and returned to us, it prints out as multiple pages. This just isn't economical. The layout of the forms needs to be more concise (the way they were turned in to you) in the way they are returned to the office. The other issue is that if the patient doesn't mark a box, the section is left blank, and we simply get no feedback. For purposes of allergies, and health problems it would probably be better if it was a yes or no question so the patient was required to answer. We have the same issue with our New patient Registration form.
2)Not all the data transfers over to Dentrix - this is an issue. If we are asking for it on the form we need to see it.
3) It was my understanding that New Patients information would automatically upload as well as existing patients. This does not seem to be the case. A smoother transfer of data for all patients would be great.

We would be 110% in for Weave Forms if it integrated directly with our patient ClipBoard with our Practiceworks software and our patient information board in our Open Dental Software. Right now we are holding off because of the lack of direct integration of patient data.

For veterinary hospitals, especially when doing curbside services, it would be very useful to be able to create and edit our own history forms.

For the Covid-19 Screening Form, its would be nice if you can marked the ones you put in the patients charts.

WEllness forms to be directly imported into Dentrix pt documents.

We really love the Wellness forms. Is it possible to create a second/different form that we could just as easily send to our patients on an as-needed basis? For example, we have to get permission from the parents for some of our scans, and it would be really helpful to be able to quickly text the permission slip, similar to the wellness form. Thanks!

We too are desperately in need of patient forms that can be filled out via text. I would be happy to help design these or help in anyway or just give feedback, test product etc.

Any progress on Online Patient Intake forms thru Weave. If not we'll go with a 3rd party provider.

Our office would also like this feature! Online Forms cut our check-in time and pre-test time by over half! Currently patients go to our website and fill it out and Crystal PM imports the info directly into the exam. If we could connect the forms to Weave either by using a short link in our auto texts or if there is a way to integrate Crystal PM and Weave with forms that would be AMAZING! We would love to be Beta testers as well if this becomes an option. Thank you!

WE Need patient FORMS for new patients and pt updates!!!! and I would to be a beta tester if possible. We just started the service and this aspect isnt covered. Also, pt photo and upload to software !! HURRY, we need Fast

We need a feature that automatically kicks out welcome emails to new patients with the ability to attach New Patient Paperwork. I had this feature with another service (revenuewell) and now I have to remember to send it to each new patient. Please add this feature!

Patient intake forms via Weave

Hi- would be a great feature to let us save our web link so we can sent to new patients so it takes then directly to our health history form.

We would like to see feature added to send out new patient forms to patients that would link into Dentrix and auto-populate.

Please add feature to be able to email or text patients new patient forms/forms needing updates. Would like them to be able to use our customized forms and sign via email/text and store directly into eaglesoft!

Automated New Patient Communication - I would like to be able to push new patients, who have made an appointment, to complete their new patient forms via our website, automatically before their appointment. Like a Welcome to the Practice, here is what to expect, please complete your forms, etc.

It would be great to have a platform for online Medical History forms. Patients could receive the form via a link in a confirmation text or email, fill out their information, then when they hit submit, it's sent back to the office. That would be simple and easy for patient and office and cut down on paper and time during appointments!
