Conditional Questions/Branching
Would be great if digital forms would allow for conditional branching or prompting (like if someone answers one question in a certain way, then an additional question/section would generate based on that answer.
Example: "Are you allergic to anything? yes/no..." answering no would allow them to keep going through the form, while answering yes would trigger another question to appear/generate. ex "please type what you're allergic to". Google forms can do it. So was hoping you can too!

Thank you for your patience and enthusiasm. I am excited to announce that we have made significant progress and recently launched the Conditional Logic feature for a select group of customers. As one of the original requesters, we have added all the offices here to our beta program. Here are the steps to follow to use this feature in the Form Editor:
Add Conditional Logic
- While editing or creating a form, find the field you would like to have conditional logic. Make sure any fields you would like to make conditional are also added.
- Click Add Condition in the bottom right corner of the field box you would like to add conditional logic to.
- Use the dropdowns to fill out the If the answer is, Then, and Field boxes.
- TIP: You can choose multiple fields to make conditional.
- Click Save Form when finished editing. You can also Preview the form in the top right corner to make sure your conditional logic is acting as you want.
The field that has been made conditional (the one chosen in the condition dropdown) will be hidden unless the patient chooses the correct answer to show the fields.
Example:
To collect insurance details only from patients who have insurance, add a Yes/No Question Element and change the title to “Do you have insurance?” Make sure to add any questions a patient with insurance will need to answer, such as Policy Holder Name, Group Number, Insurance Id, etc.
Click Add Conditions in the new Yes/No Question field. The dropdowns should look as follows:
- If the answer: Is equal to
- Value: Yes
- Then: Show
- Field: Policy Holder Name, Group Number, Insurance Id
Click Preview in the top right corner to see if the form works how you would like.

I totally agree with this. The insurance portion especially. The only way to do it right now is by not requiring the field although it should be required if a conditional response is checked appropriately!!!!!!

Certain forms need to have required answers, it needs to be filled out or the patient cannot continue to the next section.
There is a lot of information that is absolutely needed. Our main example is for Insurance Forms, if "yes" is selected for having insurance, it should be required to fill out additional information like an ID# and group #.... AND NOT BE ABLE TO LEAVE IT BLANK.
