Transfer of Documents for patients to sign
We've recently switched from Yapi to Weave. One of the features we loved about Yapi was the ability to transfer documents to specific patients from Dentrix so that they could be signed electronically. For example, when we do a treatment estimate. We would transfer the documents to Yapi for electronic signature to minimize paper usage since we are a paperless office. Once they signed the document it would go into the document center automatically.

In our new experience, we do have the capability to seamlessly attach Treatment Plans (or any other PDF document) for patient e-signature or review.
For a quick walk-through, please refer to the attached video.
To log in, follow these steps:
- Go to https://app.getweave.com/home/dashboard
- Navigate to Forms via https://app.getweave.com/forms
- Go to "My Forms"
- Select the Form that you want to send.
- Attach the PDF or Treatment Plan that you need to get signed by patients.
- Tap on "Next" and then choose either SMS or Email.
- Tap on "Send."
Patients can review and sign the documents on their device. After signing, you can view the e-signed PDFs in the submissions section and download the signed PDF document or combined file. The combined file (Form along with Signed PDF Document) is also written back to your system of record if you are an integrated office.

Would like to be able to create a consent form and then attach a treatment plan from eaglesoft as a pdf. Then be able to send in text or email to patient to sign and send back

Basically acting as a docusign is what it sounds like.
Scan existing office document, slap a digital esignature and store for retention.

Please figure out a way to have a patients treatment plan connected to a consent form so that they can sign electronically sign their consent form and treatment plan.

I am actually thinking about switching back to Yapi for this same reason. I am very unhappy with the way forms are handled in Weave. Its created so much work for our office that I really am questioning whether this move was worth it.
