12 votesWe've noticed that many of our customers' patients/clients prefer using mobile devices to fill out forms. However, embedded forms can be quite challenging to use on smaller screens and may have usability issues that negatively impact the user experience. So we highly recommend sharing the public link of the form or packet on your website. Thank you!

Send a copy of the signed forms to the patient or guardian
After a patient signs a consent form, the Weave system would automatically email the patient/guardian a copy of the consent form they just signed.
9 votesWhether it's the patient or the guardian, they can download a PDF of the form upon completion.

Pre-filled forms
It would be super helpful for the forms to be "auto-filled" with the current information that is in Dentrix. Such as name, phone number, address, etc. This saves time for the patient and will make a more pleasant experience for the patient.
2 votesNow, your patients will be able to enter their data once and have it populated in the subsequent forms in the packet -- as long as they enter the data in primary fields. The entered data will carry over if the same field is repeated across forms in a packet.
In the interest of security and compliance, we cannot auto-populate existing patient data without a username and password. We don't have any plans at the moment to build a user management system to create usernames and passwords for patients using Digital Forms.
However, we are not ruling out this functionality.

Text Popup for unentered fields
When patients fill out digital forms (health history forms for example), they are unable to proceed if they do not enter something like N/A or None for fields they do not have. This primarily is an issue with insurance information, not every patient has health insurance information. May a feature or text pop up stating "If none, enter N/A" so that they may continue submitting the remainder of the form be implemented? Thank you!
1 voteYou may make the fields optional by toggling off required. This way Forms will not validate whether the recipient filled out the field.

Multi-column layout in Form Editor
We would really like it if we could consolidate your line items on your digital forms. Ex. be able to put Fname Minitial and Lname in one line instead of 3. We would also like to be able to electronically integrate the documents into dentrix instead of having to manually enter them (for new patients)

Forms are predominantly filled out on mobile devices and single-column form layouts are best suited for mobile screens. We don't let users customize the layout to span multiple columns because it creates a poor form filling experience on mobile devices.
