Upload & Sync Patient Photos in Forms
Adding the upload photo feature to forms for new patients and have it integrate in Open dental. Nice feature to get to know your patients in advance.

Pull up patient's complete medical history so they can refer back to for updates
Requesting a way to attach patient's last or previous medical history to an update form to sign) Many patients do not remember or recall what they filled out last time. They always ask.."did I tell you X last time?, did I note X on my medical history last time?" It would be helpful to attach their medical history so they can see what updates need to be made

Mobile UI is not optimized
Forms is worthless in landscape mode because the words "Next", "privacy policy", "terms and conditions", "hipaa compliance", and "Powered by Weave" take up greater than 50% of the screen space... has anybody at Weave ever tried to fill out a form?
On the Review page for "Packets" consisting of more than 2 forms bundled, the tabs at the top to flip between the individual forms spills over into blank space on the right causing the page width to violate the scale-to-screen UI. This is very confusing for patients who are not accustomed to dealing with early 2010's mobile website design…

Setting to Disable Verification Codes for Some/All Forms
The 2FA / Verification Code system should be able to be configured to be disabled. It is unnecessary in most situations. Patient experience is fragmented now. This is exasperated by the fact that whoever designed the forms verification page coded it poorly to not allow autofill codes on MacOS Safari or iOS Safari. How is it that my code populates to autofill in the Weave Feedback Search box where it doesn't belong, but doesn't populate where it does belong....

Please allow forms to auto sinc back while patient is completing the form in case they don't submit at the end and it times out
Please allow forms to write back and sync WHILE they are completing the forms. Thus, when they are done IF they don't his submit and the form times out they don't lose everything they completed.

Preadmission Forms do not allow Canadian address
The Preadmission registration does not allow for Canadian addresses' can you please amend this. Or allow us to send a form link without the preregistration.

Thank you for your feedback! We're excited to let you know that we've started working on this feature request. The Pre Admission Registration form will soon accept Canadian postal codes and provinces. For our Canadian patients, we will rename "State" to "Province" and "Zip Code" to "Postal Code," and ensure Canadian provinces and postal codes are allowed.

Forms Verification Codes Do Not Autofill on MacOS / iOS Safari
The forms verification code box is not recognized as an input box by the system OS to assist patients in auto-filing the verification codes that Weave forces on users unnecessarily. Has anyone at weave ever bothered to use the product themselves? It is clear the form is poorly designed when the OS has no idea what it is dealing with. How did Weave dev manage to get the forms verification codes to prompt an autofill box in the feedback search box... where it has no business, but doesn't populate where it does belong....

Form links on websites
It would be awesome if we could add a New Patient Forms link to our webpage. We've noticed that often people will fill them out even before calling to make an appointment!
