  1. Pre-Admission form/ Refreshs forms

    Pre-Admission registration form screen should be able to be edited in the portal. There is a lot of information that we do not need and the patient does not fill it if there are consent forms. We fill that out since we know what the patient is coming for. Having the ability to enter what I want on the pre-admission form would be better.

    Also, when a patient is filling out forms, if they accidentally refresh the page, then it kicks them out to pre-admission form and makes us start all over. This doesn't work because patients scroll up and…

    Thanks for your patience. The registration form is a security and compliance feature designed in accordance with HIPAA regulations. Therefore, customers will not be able to edit the registration form.


    We're excited to inform you that existing patients have to fill out only four fields of information in order to access forms via Kiosk mode


    Also, starting today, any information that patients enter in the initial registration form will carry over and auto-populate into the primary fields* that are in subsequent forms in that packet. This is designed to avoid entering the same data over and over across forms in a packet, which significantly minimizes manual data entry and improves your patients' form filling experience. 


    As for accidental page refresh: the web browser will prevent users from accidentally refreshing the form page. They will see a confirmation popup like this:  "Changes that you made may not be saved."

  2. Capitalization is removed when Form is converted to PDF.

    Multiple Choice options in a form have proper capitalization when previewed on forms dashboard or when being filled out.

    When it is converted to a PDF after being completed, the capital letters change to lower case. It does not make for a professional looking document. Please advise.

  3. E-signature and Provider Review

    Hi Weave Team,

    Our office recently onboarded Weave Forms and we are hoping to have some suggestions be implemented. One feature we'd really like to see is a section at the bottom of the forms for our patients to manually sign a signature. Currently we are informed that only the electronic typed signature is possible. Another feature that we are requesting is the ability to hide certain sections until the form is completed. Ex. The section for the Dr's signature at the bottom so the patient doesn't accidentally fill this out also. Please let us know if these would be…

    E-signature options and Provider Review are generally available.


    You can add e-signature fields to your form in Form Editor > Elements. Form recipients can draw, upload an image or type their name or initials when e-signing forms.


    Also, providers can review form submissions from patients and e-sign approved forms before they writeback to your PMS.

  4. Ability to Re-order Medical Conditions

    From Scott Young, DDS, s0003509: The ability to add the "Medical Conditions" Element to a form is great. However, when we add a value at the bottom, we are not able to drag and re-order to keep it alphabetical. This is inconvenient. As of now, if we want it alphabetical we have to delete the existing fields or re-type them all. We want to add other conditions to match what we record in our PMS. But if we add, say Anxiety. It's stuck at the bottom.

  5. Populate Primary Fields in Forms with Existing Data

    Now, your patients will be able to enter their data once and have it populated in the subsequent forms in the packet -- as long as they enter the data in primary fields. The entered data will carry over if the same field is repeated across forms in a packet.


    We cannot auto-populate existing patient data without a username and password. We don't have any plans at the moment to build a user management system to create usernames and passwords for patients using Digital Forms. However, we are not ruling out this functionality.   6. Forms pdf importing

    PDF of forms is not importing into Open Dental. Please fix this ASAP! What's the point of having these if they literally are not working?

    why are the forms STILL not integrating properly with open dental? I was told this was going to happen LAST JUNE. Need med history and allergies to import.

  8. Download submitted forms properly

    On the portal- it doesnt let you download the forms, it only lets you print them to retrieve the patients submitted form.

  9. Show Patient Name for Auto-Sent Forms

    On the Admin Portal, under the Forms Dashboard, in Sent Forms List - when a form is auto-sent (in our case, our custom wellness screening to patients every day) it doesn't display the Patient Name. It would be helpful to be able to see it, so at a glance you can see who has/hasn't submitted the form.

  10. Weave Forms - Not a happy client

    Weave Forms ---- I would like to let you know that we are recent clients to the weave forms. We are NOT impressed. We are having little to NO feed back on the forms. And when look into the forms section to your website there is little data available so I can research the issues. I have contact weave a few times and spoke to representatives about this issue... and NO one got back to me. I have noticed the names now appearing but other issues were not being addressed, and again no calls back. So now I have noticed…

  11. Weave forms packets

    Our Weave Digital form packets are coming back as individual notices on our dashboard but everyone form in the packet is in each notification on the dashborad. Our packet has 6 forms in it, so we are getting 36 forms for the 6 forms. If we don't send a packet then the patient get 6 text messages to fill out each individual form. This has to be fixed STAT! also we HAVE to be able to delete that forms we've downloaded on the dasboard so we know it been taken care of. HELP PLEASE!!

  12. Digital signature field in Form Editor

    FINALLY you guys fixed the new patient paperwork so it isn't 36 pages... but it is still spaced out wayyyyy to much.. it needs to be a lot closer together so it isnt 10 pages.... I need someone to respond to me ASAP about my request. Also- digital signatures need to be added to all forms.... please email at info@montesperiodontics.com or call our office at 770-781-3685.

  14. Forms need to be able to be edited

    The forms you made for our office are too spaced out and the text is too big. This cannot be changed or edited even by support. We will be cancelling this service due to this inconvenience

  15. Mark reviewed forms as READ

    It keeps kicking me out every time the phone rings and not able to finish idea. When forms are done it would be nice to mark it as read or reviewed so it doesn't mean double work having 2 different people reviewing

    The Digital Forms team is pleased to announce the release of Submitted Form Notifications--our most requested feature. As part of this feature, all unread form submissions can be identified by a distinct dot which disappears as soon as you're done reviewing the form. In a multi-user office, the user reviewing a form first has a Mark as Unread option in the form to keep it new and relevant for other users reviewing form submissions. We hope you enjoy using this feature.

  16. Improve the layout of PDFs

    The forms PDF is WAY WAY WAY to far spread out. Please condense the forms. Also, when patients are filling out the forms and they click to go to next page to continue filling it out, it automatically goes to the bottom of the next page. This is causing patients to skip many questions easily and not filling them out properly. Must scroll back up to next page.
    - Some patients are having issues with getting the link to work on phone or computer and some are not, is this an issue for anyone else ?

  17. Order of form packets

    With forms, we created a packet with 3 different forms. However, when we receive it, it's in reverse order and we REALLY need that first one to be first.

    can we condense the docs as well? Lots of white space.

  18. forms and support

    When it comes to patient forms we are getting different information from the representatives. We were told that "patient profile information" would transfer into dentrix however it isn't. we had someone trying to help us for about 3 hours last week and never got a return phone call. i called again today and was told it isn't possible. we left our previous provider due to always getting inconsistent information. I personally do not see how these electronic forms are beneficial if they only transfer into the weave profile as we have to have the information with in our system so…
