Pull up patient's complete medical history so they can refer back to for updates
Requesting a way to attach patient's last or previous medical history to an update form to sign) Many patients do not remember or recall what they filled out last time. They always ask.."did I tell you X last time?, did I note X on my medical history last time?" It would be helpful to attach their medical history so they can see what updates need to be made

Improve Kiosk Mode
For OpenDental offices, it would be helpful to be able to (optionally) provide the OpenDental Patient Number as the key for Existing Patients, rather than First & Last Name, DOB and Mobile phone number. As an aside comment for consideration - not every patient has a mobile number (most do, so it not that big of an issue).

Doctor review and approval feature
There should be a feature/setting that allows you to select a particular form (such as medical history) for doctor review. When patient complete form it would enter a queue for the doctor to review and approve later. Also, why isn't the forms icon/option available on the mobile app? Development of digital forms feature seems rushed.

Easily compare and update (sync) mismatched information
I want to be able to delete a from from needs approval if I choose not to use it or don't want it uploaded.
If we send a form to a parent and they fill out the data for their child it appears under the parents name so if we sync it won't go to the child it will change all of the parents info.
Also there should we an alert to any mismatched information so that it we can compare and update before syncing.

Please use the Archive feature to archive any form regardless of its status,
Please make sure that you are using Primary Fields correctly. To ensure accuracy of writebacks, name fields intended for collecting the patient name should not be used in any other form within the same packet to collect guardian or other information.
Tip: You could use Short Text Entry (available in Form Builder > Elements) for collecting names of anyone other than the patient.
We are working towards providing an easy way to compare submitted data with previously synced data at the field-level before write back. We'll keep you posted when this feature is ready to go live.

Select/confirm patient when doing a manual sync
We post a link to our forms on our website. When a patient calls to schedule an appointment we aske them to fill out the forms on our website. However if they put a different name, for example William vs Bill or the put a Jr vs nothing it will create a new patient instead of connecting it to the patient we already created. It would be nice when doing a manual sync if we could confirm the action and\or select a different patient to sync to

Ability to duplicate digital forms
Request for the ability to duplicate forms instead of recreating an entire form new form. For example, our previous patient forms are only slightly different from new patient forms. It would be helpful to have the ability to duplicate forms for minor changes.

"No To All" option for medical conditions on registration form
Is it possible to have a "no to all" button? We have about 20-30 listed medical conditions on our registration forms. Clicking "no" so many times is tedious and time consuming. Patients are stopping midway through. It'd be great to have a "No to All" button for listed medical conditions.

Select which froms for Manual review and write back
When we switched from mConsent to Weave forms we lost the ability to have the provider electronically sign the from in their own portal; if weave would allow an option in the Form Editor for the office to select which forms could be manually reviewed the provider could then electronically sign it before it is written back.
Post COVID I am surprised that we have the ability to have it on the patient phone yet ask the provider to touch the patient phone to electronically sign.
Right now if we make all forms manual we loose the benefit of which…

Highlight patient responses in Forms
Hi,
In PDFs generated by digital forms it is not easy to find patient typed responses since the fonts are almost all the same color.If you could change the font color for the Pt typed responses and all radio buttons of yes to the yes/no questions, it will be much easier to scheme and read the resulting PDF file in the system.

Can we please make a Ipad app where patients can sign the forms?
With Mconsent patient signed forms on the Ipad and they uploaded to the patient chart as soon as they were done signing can this be done?

Have your tried using the Kiosk Mode in Digital Forms?https://www.weavehelp.com/hc/en-us/articles/4561323526427-Use-Kiosk-Mode-in-Digital-Forms
1) You could login to the Forms portal on your iPad and ask your patient to fill out the forms or 2) ask them to scan the QR code to fill it out on their personal mobile device.
Depending on your writeback settings, Forms submitted via Kiosk mode will automatically sync to your PMS. It will also attach the PDF to your patient's record in your PMS.

Allow for customization/control of where submitted forms populate in the document center (Dentrix, OpenDental, Eaglesoft)
This would allow forms users to specify where documents are populated in their document centers and greatly enhance the usability and organizational structure of submitted documents. It would be helpful to be able to specify where each form is deposited and allow it to be as granular as possible. This would be a great quality of life improvement for forms engagement post-submission of forms and would make locating and accessing submitted forms much easier.

Referral Source Field
Buxton Dentistry, s00061705, would like to keep Referral Source as a Primary Field that can writeback to Open Dental. Please advice.
1 voteReferral source is now available as an Elements field in Form Editor. It comes with preset options that can be edited -- please refer to the attached screenshot. However, it does not writeback to the system of record.
As of now, we don't have a definite timeframe on when Referral Source will be available as a Primary Field. We'll keep you posted as soon as it's ready for development. Thanks for your valuable feedback!

Designating Document Groups to Use Intellicare (in Eaglesoft with Weave Forms)
We use Weave and Eaglesoft. When patients complete a Form through Weave, it imports into our SmartDocs in Eaglesoft in the form of a PDF and the PDF gets placed into a "Patient Identification" document group category.
We would like to use Intellicare(Eaglesoft feature) to let us know when patients need to update their health history(for example).
In order to use Intellicare for this purpose, we would need our Health History Forms to go into a specific document group and other forms to go into a separate form (instead of ALL forms going into the "Patient Identification" group.I have…

I would like to see Weave Forms integrate with Compulink. In The winter of 2021, I took a forms demo and at that time was told that Forms integrations was scheduled for June 2021. When will this be available? I am starting to demo other products that are already integrated.....

NP Form Packets
Having the ability to send out multiple forms in one text is great. However when we get them all back there are now just one file and we cannot piece out individual forms to place them in different areas of Dentrix. It would be nice if they could still be individual forms. I don't want to send 6 different texts just so I can keep the forms separate.

digital forms patient name changes
When patients fill out and submit digital forms, however they fill out their name on the forms automatically changes their name from our software. For example, patient's name in the our software is John Doe. but if patient types in Joseph Smith, then it will automatically erase John Doe from our software and puts in Joseph Smith, thus no longer we are able to locate the patient in our software. I understand that patients may make mistake when typing in their names or they may put in their nickname instead of full name, thus may create an error. I request…

Improve the layout of forms created in Form Editor
As far as the forms go, it would be great if the editor had control of the spacing as the documentation is very choppy and spacey. Also, if you can find a way that customers can logo their forms in specific places not just centered at the top of the document in font 48 size. A way to make them prettier and more eye catching would be ideal. Thank you.
