Hi! We send out 8 forms in our New Patient packet (all individual forms and all individually named), but they are coming back as one single giant form. It would be so much better if the forms saved using their "title" as listed in the packet. It's too time consuming to send these forms individually to each patient and too difficult to try to pull out a form of the giant single form returned to us. Please consider making this adjustment.
