digital forms patient name changes
When patients fill out and submit digital forms, however they fill out their name on the forms automatically changes their name from our software. For example, patient's name in the our software is John Doe. but if patient types in Joseph Smith, then it will automatically erase John Doe from our software and puts in Joseph Smith, thus no longer we are able to locate the patient in our software. Individual PDFs in Dentrix Doc Center
Hello. When we have a new patient complete our new patient pack of forms, we'd like there to be a better way for this to sync with Dentrix. Currently all the forms are in one pdf file in document center and it's not a very user friendly way of doing this. Each form should be put in separately so someone can easily access the medical history or dental history for example. Right now with all forms being in one large pdf file it is not easy to just look up a patient's medical history. Also the medical conditions allergies or…

Specify a folder in the PMS to sync PDFs
Please create an option to choose which folder in the PMS forms are saved to/in.

Eaglesoft Patient Medical Alert
On the medical history form, when a patient has an allergy or medical condition, it should automatically go into the patient's echart as an alert in Eaglesoft.
On the Eaglesoft Medical History form, Drug Addiction should be changed to Drug Dependency Disorder

Designating Document Groups to Use Intellicare (in Eaglesoft with Weave Forms)
We use Weave and Eaglesoft. When patients complete a Form through Weave, it imports into our SmartDocs in Eaglesoft in the form of a PDF and the PDF gets placed into a "Patient Identification" document group category.
We would like to use Intellicare(Eaglesoft feature) to let us know when patients need to update their health history(for example).
In order to use Intellicare for this purpose, we would need our Health History Forms to go into a specific document group and other forms to go into a separate form (instead of ALL forms going into the "Patient Identification" group.I have…

Patient pictures
It would be great to have a place where we can take a picture of the patient where it directly syncs with our practice management system and into the weave dashboard. Currently we are using the "card capture" feature to do this and then the snipping tool to cut out their picture to import in, but it asks for a picture of the front and back of the card so when using this feature the patients think we are asking for a picture of the back of their head.

I use Patterson Fuse, if appointments confirmation information can be synced to the PMS directly, why can't the form information be done too. Ideally personal information/updates, payments applied directly to the ledger, and stretch is health history.
As an aside, Revenuewell has had the direct write back sync for more than two years for info/hh. I understand cost/benefit from a userbase perspective, but i'd ask.

NP becoming active
When New Patients fill out the forms it should not make them an active patient. We should make them active when or if they come in the office.

Select/confirm patient when doing a manual sync
We post a link to our forms on our website. When a patient calls to schedule an appointment we aske them to fill out the forms on our website. However if they put a different name, for example William vs Bill or the put a Jr vs nothing it will create a new patient instead of connecting it to the patient we already created. It would be nice when doing a manual sync if we could confirm the action and\or select a different patient to sync to
