Preadmission Forms do not allow Canadian address
The Preadmission registration does not allow for Canadian addresses' can you please amend this. Or allow us to send a form link without the preregistration.
1 voteHi Liz,
Thank you for your feedback! We're excited to let you know that we've started working on this feature request. The Pre Admission Registration form will soon accept Canadian postal codes and provinces. For our Canadian patients, we will rename "State" to "Province" and "Zip Code" to "Postal Code," and ensure Canadian provinces and postal codes are allowed.
We'll keep you updated on our progress and reach out if we need any additional input from you.
Thank you again for your valuable feedback!
Best regards,

PCI DSS compliance to enable secure capture of CC info in Forms
PCI compliance so that you can capture CC information on a patient's form for secure storage in PMS. Competitors are doing this and I would love to see Weave get this done soon!
13 votesThank you for your patience. I’m Mayur Pathak, Product Manager for the Digital Forms product. I’m excited to share an update on a feature we’ve recently started working on. Soon, we'll enable the capability to send a Form Link to patients via email, SMS, or kiosk that will include a Stripe payment field that allows patients to securely save their credit card information. That Payment field could be added to a form via our Digital Form Builder.
With this feature, patients can conveniently fill out the form at their own pace, and their card information will be securely stored by Stripe in a PCI DSS-compliant manner.
We’re looking forward to the beta launch in the coming weeks and would love for you to join as early adopters. If you’re interested in participating or have any questions or suggestions, please reach out to me at mayur.pathak@getweave.com. I’d be happy to discuss further…

Improve Kiosk Mode
For OpenDental offices, it would be helpful to be able to (optionally) provide the OpenDental Patient Number as the key for Existing Patients, rather than First & Last Name, DOB and Mobile phone number. As an aside comment for consideration - not every patient has a mobile number (most do, so it not that big of an issue).

Easily compare and update (sync) mismatched information
I want to be able to delete a from from needs approval if I choose not to use it or don't want it uploaded.
If we send a form to a parent and they fill out the data for their child it appears under the parents name so if we sync it won't go to the child it will change all of the parents info.
Also there should we an alert to any mismatched information so that it we can compare and update before syncing.
1 voteDear Customer,
Thanks for your valuable feedback.
Please use the Archive feature to archive any form regardless of its status,
Please make sure that you are using Primary Fields correctly. To ensure accuracy of writebacks, name fields intended for collecting the patient name should not be used in any other form within the same packet to collect guardian or other information.
Tip: You could use Short Text Entry (available in Form Builder > Elements) for collecting names of anyone other than the patient.
We are working towards providing an easy way to compare submitted data with previously synced data at the field-level before write back. We'll keep you posted when this feature is ready to go live.
Thanks,
KJ

Highlight patient responses in Forms
Hi,
In PDFs generated by digital forms it is not easy to find patient typed responses since the fonts are almost all the same color. If you could change the font color for the Pt typed responses and all radio buttons of yes to the yes/no questions, it will be much easier to scheme and read the resulting PDF file in the system.
Please let us know if that is something that you can do for us.
Thank you42 votesDigital Forms PDF Improvements
We've improved the way PDFs are generated and presented in Digital Forms. Here's a summary of the notable updates:
- The PDF header now displays your logo or business name, along with important patient details.
- Page numbers in the footer ensure usability for both digital and print formats.
- Optimized spacing and text sizes enhance readability.
- Bold text distinguishes patient responses from questions.
- Highlighted YES answers (in fluorescent yellow) aid quick scanning and identification of crucial information.
- A 4-column layout for checklists ensures a streamlined design.
- Optimized page margins display more content per page without sacrificing readability.
We've made these improvements based on valuable feedback from amazing users like you. If you don't see these updates, please let us know. They will be generally available later this month.
Also, please take a moment to rate this update and continue to help us make this product better.
Thank you…

Improve the layout of forms created in Form Editor
As far as the forms go, it would be great if the editor had control of the spacing as the documentation is very choppy and spacey. Also, if you can find a way that customers can logo their forms in specific places not just centered at the top of the document in font 48 size. A way to make them prettier and more eye catching would be ideal. Thank you.

Permissions to access Forms
Some besides Admin needs to be able to access forms. Maybe the Advanced team member.
