  1. Additional info request

    It would be very helpful if a line was added for a phone number for the patient's insurance carrier (s) or at least made an option to add/edit.

  2. Guardian Field in Forms

    *What department are you in? * Support

    *Which Feature does this pertain to? * Forms

    *What area of the Feature? * Forms Creation/Primary Fields

    *What pain points/suggestions does the office have?
    * The office would like to have a primary field (or two) for Guardian's information, as a lot of their patients are underage.

  3. Dual Signers

    Ability for patient to complete form, then the doctor is able to sign or someone from the office.

  4. Referral Source Field

    Buxton Dentistry, s00061705, would like to keep Referral Source as a Primary Field that can writeback to Open Dental. Please advice.

    Added to Backlog  ·  AdminKJ (Group Product Manager, Weave) responded

    Referral source is now available as an Elements field in Form Editor. It comes with preset options that can be edited -- please refer to the attached screenshot. However, it does not writeback to the system of record.


    As of now, we don't have a definite timeframe on when Referral Source will be available as a Primary Field. We'll keep you posted as soon as it's ready for development. Thanks for your valuable feedback!
