digital forms patient name changes
When patients fill out and submit digital forms, however they fill out their name on the forms automatically changes their name from our software. For example, patient's name in the our software is John Doe. but if patient types in Joseph Smith, then it will automatically erase John Doe from our software and puts in Joseph Smith, thus no longer we are able to locate the patient in our software. I understand that patients may make mistake when typing in their names or they may put in their nickname instead of full name, thus may create an error. I request that whatever names they put in into the digital forms do not interfere nor change anything in the software. thank you.

Does changing the writeback setting to "Manual" for "Patient Record Updates" help with this problem? We're finalizing our digital forms and I'm trying to avoid running into this issue!

We are finding this too, especially with nicknames vs. formal names (which we have to have in the system for insurance billing). Is there a way to include the nickname field of our EHR (Crystal PM) as part of the match along with first name? This would save us a lot of manual work and avoid a new patient being created unnecessarily!

I just signed up for Weave yesterday and I already found this to be a problem. It makes absolutely no sense why a second record is created for a patient if they update their name or phone number, especially since most people change their name at least once in their lifetime and their phone number at least 10! This would be crucial to correct because I have already ran into the problem of not being able to find the patient myself.
