Teeth Chart
It'll be great to have a set of checkboxes representing the teeth numbers. 1-16 on top and 17-32 on the bottom. That way when we're doing a consent form for extractions. Our assistants can select the teeth #s that we are working on. Also have the teeth #s in the order of how we think of teeth. So 1-16 on top line, and 32-17 on lower line.

Text/Email form to patient not in system yet.
Can we make it so we can Text/Email a form to patient that is not in our system yet? That way we can have new patients essentially create their own patient account in our system.

Dashboard organization
Can there be sub categories in the inbox field for every form template? Everything is just grouped together and I think it would like nice to have Weave recognize what form was filled and organize it into it's own designated folder.

Doctor review and approval feature
There should be a feature/setting that allows you to select a particular form (such as medical history) for doctor review. When patient complete form it would enter a queue for the doctor to review and approve later. Also, why isn't the forms icon/option available on the mobile app? Development of digital forms feature seems rushed.

Select which froms for Manual review and write back
When we switched from mConsent to Weave forms we lost the ability to have the provider electronically sign the from in their own portal; if weave would allow an option in the Form Editor for the office to select which forms could be manually reviewed the provider could then electronically sign it before it is written back.
Post COVID I am surprised that we have the ability to have it on the patient phone yet ask the provider to touch the patient phone to electronically sign.
Right now if we make all forms manual we loose the benefit of which…

Designating Document Groups to Use Intellicare (in Eaglesoft with Weave Forms)
We use Weave and Eaglesoft. When patients complete a Form through Weave, it imports into our SmartDocs in Eaglesoft in the form of a PDF and the PDF gets placed into a "Patient Identification" document group category.
We would like to use Intellicare(Eaglesoft feature) to let us know when patients need to update their health history(for example).
In order to use Intellicare for this purpose, we would need our Health History Forms to go into a specific document group and other forms to go into a separate form (instead of ALL forms going into the "Patient Identification" group.I have…

I would like to see Weave Forms integrate with Compulink. In The winter of 2021, I took a forms demo and at that time was told that Forms integrations was scheduled for June 2021. When will this be available? I am starting to demo other products that are already integrated.....

I would like for weave to implement a "clone" for packets and not just for forms. I think that would be more efficient. I chatted with weave support today and they stated cloning packets was not an available option at this time.

Matrix Question for Weave Forms
We have several symptom questionnaires for patients and need a MATRIX question option that can calculate a symptom score. Example: Symptom #1: 0 points for never, 1 point for seldom, 2 for sometimes, 3 for often, 4 for always. This continues for a list of 20-30 symptoms.

Hide conditional logic on complete form
I love online forms and using this conditional logic, but is there anyway to hide the "unanswered" questions on the completed form? It just seems unnecessary to show everything that is on the form, if you have a lot of conditions.

Practice Websites and Weave Interaction
While rebuilding our website, I wanted to use Weave interactions to allow patients to interact with text chat with our office from our website via code or a widget. I called, only to find out that there is a plan I can purchase for this. Honestly, dumb idea! If someone is willing to incorporate Weave widgets in their website, or via hard code, what are the chances they will ever cancel Weave service? NONE! ...that's the answer. Give away widget interaction for websites and keep Weave customers for life!

Patient pictures
It would be great to have a place where we can take a picture of the patient where it directly syncs with our practice management system and into the weave dashboard. Currently we are using the "card capture" feature to do this and then the snipping tool to cut out their picture to import in, but it asks for a picture of the front and back of the card so when using this feature the patients think we are asking for a picture of the back of their head.

Eyefinity Integration
We would love to see the forms integrated with Eyefinity PMS where the forms write back to Eyefinity.

Please allow forms to auto sinc back while patient is completing the form in case they don't submit at the end and it times out
Please allow forms to write back and sync WHILE they are completing the forms. Thus, when they are done IF they don't his submit and the form times out they don't lose everything they completed.

form order
The list of forms is not in any particular order, not even alphabetical. It would be great if we can stack them the way we want, for us it would be great if we could put on top the ones we use the most. When we had only a few forms it was fine. As we are adding more and more we have to go through the whole list. If we can not select a custom order, at least alphabetical or numerical order. Thanks.

Form Errors
I am constantly getting errors when patients are trying to fill out their New Patient Packet (we have a registration form, Auth to release records, Hippa and Eaglesoft Medical History) it always happens with the medical history. I have spoken to support multiple times and they have given me multiple fixes none last. The main thing they tell me to do is sync the Eaglesoft Medical History but at this point we are doing it multiple times a day and patients are still getting errors. Weave forms integration with Crystal PM Record page
Weave currently integrates digital forms with Crystal PM by uploading a PDF copy of the filled out form into the patient chart. It would be highly beneficial to have the ability to transfer over the filled out information directly into the patient's record page to avoid the need for double data entry, so we currently use Crystal's online form. Will sign up to Weave's forms in a heartbeat once that feature is available due to much more flexibility with customization and seamless interface for the patients.
