  1. Be able to recolate Chat Widget

    In a Squarespace website our cookie banner can only be in the footer or the header. It's not pleasant to have in the header and that is where we have our Promos running.

    Your Chat Widget then overlays on top of the Cookie confirmation Notification "OK or "Decline."

    The script should be written to modify allowing relocation.

  2. Customize text post Booking Form submit

    After submitting a Booking form online the messaging is very vague. "Can't wait to see you" is misleading to the patient that their appointment request was a "Success!" and they will likely start showing up to our offices.

    The "Success!" message as well as the "Hi! We got your appointment request and will confirm soon. Can't' wait to see you!" should all be customizable.

  3. Send as a packet, but forms should come back individually named

    Hi! We send out 8 forms in our New Patient packet (all individual forms and all individually named), but they are coming back as one single giant form. It would be so much better if the forms saved using their "title" as listed in the packet. It's too time consuming to send these forms individually to each patient and too difficult to try to pull out a form of the giant single form returned to us. Please consider making this adjustment. Thank you!

  4. Auto-calculate total score based on patient responses

    Our office has a form that requires patients to check from "Never, Seldom, Occasionally, Frequently, and Always" in about 20 multiple choice questions. Each answer is worth a certain number (i.e. Never=0, Seldom=1, etc) and we currently have to manually grade and calculate final score. It would be nice for the form to automatically calculate the score based on the responses from the patient to save time doing that after the fact.

  5. Medication list is long as patients go through 10 pages of medications to select one

    Every time our patients update their medication list they have to go through 10 or more pages and select their certain medication. We have a lot of feedback on how hard this is. If you could have 26 tabs (each letter of the alphabet) as a dropdown to the med list then that would help. What would even be better is to have them start spelling the medication and the options would start populating so they could easily select their meds. In the days of AI this should be easy to do. Make us all happy and make it easy…

  6. Forms not being sent to patients with shared contact information

    We have run into an issue with patient's who come in with their dependents or they share a phone/email with another, only one person receives the paperwork. I understand that I can manually send forms to each person, but this does negate the point of the automated messaging system. I hope in the future the system would be able to send out individual messages addressed to each scheduled patient regardless of them having the same phone number.

  7. Digital form update

    When patients are completing a medical update and it syncs to Eaglesoft. Please change it to add to the medical update, as opposed to deleting any existing medical information.

  9. Form Templates

    Looking to hopefully get it that theres no limit on "mobile phone number" spots that we can put into a spot and that it automatically puts it into the format of (123)456-7891. Also for email addresses to have no limit on there and make sure that its a proper email address.
    Also for us Canadian customers, instead of "state" and having it show all of the states, can we have a province one that shows all provinces??

  10. Preadmission Forms do not allow Canadian address

    The Preadmission registration does not allow for Canadian addresses' can you please amend this. Or allow us to send a form link without the preregistration.

    Thank you for your feedback! We're excited to let you know that we've started working on this feature request. The Pre Admission Registration form will soon accept Canadian postal codes and provinces. For our Canadian patients, we will rename "State" to "Province" and "Zip Code" to "Postal Code," and ensure Canadian provinces and postal codes are allowed.

  11. Patient ID in Kiosk Mode!!

    I would love to see an option when looking up patients on Kiosk mode to type in the Patient ID number rather than the lengthy process of name phone number and DOB, I have had one patient who was having trouble with the forms some how end up on someone else's acct which was a disaster. With PT ID numbers we as a front office could ensure the patient is filling out the proper forms.

    I wish we could choose who the forms go to. It always sends to the guardian. When you want to send to the spouse it will usually go to the husband who doesn't fill it out. And the kids will go to him.

  13. Health/Medical History: 1 Question With Boxes to Check/Uncheck Instead of Separate Questions For Each Condition

    It would be much easier (less tedious) for the patient, on the health history part of a digital form, if instead of individually asking 100 questions about their medical conditions that they have to click yes/no to, to just have one question with all the boxes of medical conditions that they can select all that apply. So they can either just check 1 box that says None of the Above, or just click the couple of boxes that correspond to the medical conditions they have, instead of having to tediously click yes/no to 100 different medical condition questions.

  16. Hide information in completed form

    It would be useful to be able to hide information from the completed form. For example, we have a price list section on the form, which is information that is useful for the client, but not for office use. Being able to hide the unneccesary info would speed up workflow, and help condense the forms.

    We asked specifically of our sales Weave Rep if Weave had an App for the Kiosk I Pad and were told yes. Unfortunately this is not true. There is no App. An App would make checking in patients much more stream lined and easier to check in patients for forms and new patient information.

  18. Multiple choice conditions unselecting previous answer

    When creating a condition within a multiple choice, selecting the option that is related to the condition opens the second multiple choice section as expected, but it then deselects the previous answer.

    For example:

    Multiple choice with A, B, and C.

    When C is selected, another multiple choice option appears, (condition) options D or E. This is what we want.

    When D or E is selected, C is deselected. For record keeping's sake, we need Weave Forms to indicate C was selected, as well as D/E.

    Upon further review, it looks like this issue is also happening when having seperate…

  19. autosignature

    You should have a place where we can paste autosignature for regular staff. This why the staff does not have to sign the form each time we get it. We can pick from one of the 10 staff and paste the signature to the box.

  20. Confusing verbiage on sent forms

    In the app, under sent the name of the packet that will be sent out is the the actual packet. They are all listed as "office visit Packet" which is the name of a different packet. Once it queues up the patients to send the reminder to with a form it then reads the filters to send the correct packet.
