Dentist signing forms
If it requires the dentists signature, why do I have to touch the patients mobile device to do so? Why are they not able to sign then send then I sign and approve? I am really unhappy with these forms and am starting to feel like I should have just stuck with Yapi.
1 voteHi Dr. Maria, If you have setup provider review in Forms, users with Forms Manager or Provider role can review and eSign submitted forms anytime anywhere as per their convenience.

Delete multiple fields simultaneously
There should be a way to delete multiple fields at the same time. Especially when cloning forms as templates, sometimes you want to delete an entire section and you have to delete. each. field. one. at. a. time.......super time consuming =/

Text/Email form to patient not in system yet.
Can we make it so we can Text/Email a form to patient that is not in our system yet? That way we can have new patients essentially create their own patient account in our system.

Show which user printed out the patient forms
It would be helpful for a large practice if it would show which user printed off the patient forms. For tracking purposes.

Form links on websites
It would be awesome if we could add a New Patient Forms link to our webpage. We've noticed that often people will fill them out even before calling to make an appointment!

Adding the upload photo feature to forms for new patients and have it integrate in Open dental. Nice feature to get to know your patients in advance.

Eaglesoft Patient Medical Alert
On the medical history form, when a patient has an allergy or medical condition, it should automatically go into the patient's echart as an alert in Eaglesoft.
On the Eaglesoft Medical History form, Drug Addiction should be changed to Drug Dependency Disorder

Forms in Bulk Messaging
If there was a way to send forms in bulk messages, this would increase our productivity. We want to be able to send out ins update forms in the beginning of each year.

Improve the terrible layout of PDF Forms
We are very unhappy with Weave forms, the concept is great but the format that is returned to us make more work for us. For more information please contact me directly, we will be calling and cancelling our subscription for this service. :(

Treatment Plans for Patients to sign
Treatment plans that integrate with the Sunbit option to pay. Having the option to create treatment plans at all would be nice.

NEW PATIENT FORMS
I tried to use your desktop app but the login process didn’t work!
Is there a way to send one link to complete multiple new patient forms for family members? Our current set up only allows one patient to complete the form when sent to the same cell and/or e-mail address. When they attempt to do the next family member, the system tells them they already completed the form. I hope this makes sense to you.2 votes -
Dashboard organization
Can there be sub categories in the inbox field for every form template? Everything is just grouped together and I think it would like nice to have Weave recognize what form was filled and organize it into it's own designated folder.

Weave Forms - Need to eliminate first page for Dentist Clearance Forms
Our dentists like to send patient referrals via our website.
We want that referral process to be HIPAA compliant AND
we want it to be as easily as possible.That means we DO NOT want the on boarding page to be there. It is an extra steps for the dentist, it transfers the dentist information into the referral and we DO NOT want to loose referrals because we have a CLUMSY and CUMBERSON process to refer patients.
Easy is BETTER!

PCI DSS compliance to enable secure capture of CC info in Forms
PCI compliance so that you can capture CC information on a patient's form for secure storage in PMS. Competitors are doing this and I would love to see Weave get this done soon!

Pull up patient's complete medical history so they can refer back to for updates
Requesting a way to attach patient's last or previous medical history to an update form to sign) Many patients do not remember or recall what they filled out last time. They always ask.."did I tell you X last time?, did I note X on my medical history last time?" It would be helpful to attach their medical history so they can see what updates need to be made

Integrated with Dentrix medical alerts so that existing medical alerts prepopulate in the Weave Form - and any updates the patient makes are then written back into the Dentrix medical alerts fields.

Sending Weave Forms
Hi there,
I'm having some trouble when sending our Weave forms. There have been a few instances where I send out a form to Jon Doe and when the message populates with the form link, Jane Does name is shown. This happens with a few family members. I don't know if this is a Weave setting I can remove or edit? I notice when texting a spouse (John Doe) sometimes the other spouses name (Jane Doe) auto populates and displays it on top of the weave thread, even though that have different cell phone numbers. Editable Forms Link Text Message Template
Currently there is a pre-populated template that comes up when clicking on the clipboard icon within a message thread. Offices would love to be able to edit the template to adjust the wording as they wish, like they are able to do with other message templates within the desktop client.
