What pain points/suggestions does the office have?
I've seen this with several offices, this is just the most recent:
We need two part forms (especially for Dentist offices that do extractions). So they would have, for example: a template for teeth extraction. Then they would send the form to themself and they could fill out the tooth#, reason for procedure, bp, etc. Then forward it to the pt for final signature and anything else they want the pt to fill out on the same form. So they could customize that form to the pt.
