I am looking for a feature such as an email or text in letting me know that a new patient forms have been completed.
Under the sent forms tab there needs to be some sort of check mark system for ones that have been completed and then imported into our software. When there are multiple front desk working on weave we have no idea which forms have been transferred into the patients chart and which ones need completed. It would also be nice to add a button where we can hit resend form if the patient still has not completed it.

We are a dental office and we liked the question on the Wellness Form if patient has been vaccinated. Wondering why it was removed and if we can get it back. Thanks
CDA has issued new screening form for dental offices. Can you please match the form they have so we're in compliance?

We see that you added then removed the "have you been vaccinated" question. Along those lines..Why not eliminate the question..Are you over the age of 60. So irrelevant
I need VACCINATION off the Wellness Form.
On our Covid wellness Form, there is now a question re: if pt is vaccinated. Here in Canada it is against our law to ask these questions. Can you please delete it from our Pt form?
I was very disappointed to learn that a question was added to the wellness form without our office being notified. We had a patient complain about the vaccine question and were VERY embarrassed because we were not even aware that question had been added. In the future if there are things added to a form that offices are sending to patients it would behoove you to inform the offices about this!!

Please get remove the question asking if our patients have been fully vaccinated on the wellness form. Also an email stating the updates that will be changing forms sent out to patients would be appreciated.

Please help address the forms completion return area. In addition to seeing patient names, we need to move off forms that we've downloaded and placed into the patient document. At present, they stay forever. It will be a full time job in a pedo office to determine which forms have been reviewed and uploaded verses all of the incoming forms. So we were onboarded for FORMS yesterday. After playing around in the program, we noticed several measures that we had questions about concerning the capabilities that became familiar with when using forms with a previous company.
I would love help with the following:
1. How do we delete files returned from patients that have the wrong or incomplete information?
2. How would we organize the packet to be received by the patients in a certain order? i.e. Patient Info Form, Health Form, Notice of Privacy then Covid-19 Form (in that order)?
3. We have 4 Forms in the packet. When the Forms were returned, we received 4 files containing 4 sets of the same Forms.
4. How do we get the file to show the Patient's name upon return? It shows as a file number.
5. Can Patients go back into the forms if they don't finish them? or do we have to send them another set of forms?
6. How do patients go back to the previous page in the forms if they would like to see their answers, change info or edit any past pages?
7. Will previous forms auto-populate if we send the form again? say 6 six from now?

Please delete the question about travel from the wellness form since it's mostly does not apply anymore (especially for Illinois)

Please update the covid questions. Can you take away the question have you traveled? It is irrelevant since every where is affected by covid.
Covid Screening Wellness Form - Questions no longer relevant we need ability to edit forms or you guys need to update it.
Now most important is to ask if patients have completed their COVID vaccines.
Can we add on the wellness form about if they had completed the vaccine shot and date? This would help us make the wellness form more useful as the mask mandates are changing.
The Wellness form has not been update in a year. I would like to see additional questions on the form. The more we know, the more when know to keep our staff safe. Maybe: Have you had Covid? Have you had the Covid Vaccine and which one. We have taken so many precautions but we want to stay informed without a dozen additional forms. We are not interested in changing the way we send our New patient forms to patients through Yapi only the Wellness form for now. Thanks again. Lee

We've had constant questions from patients about the wellness form...in particular regarding the last question about traveling, which seems to be irrelevant, and they wonder why we don't ask if they are vaccinated. That may be a good replacement question. Please consider updating them! Thank you!!

The CDC updated its mask and COVID guidelines May 11 2021. Can you please update the WELLNESS COVID form to reflect these changes? For example, it should ask if people have been vaccinated prior to entering the office. And travel guidelines have changed. Thank you
