UPDATE WELLNESS FORMS TO ASK ABOUT VACCINATIONS, WHICH VACCINATION, WHICH DOSE, AND WHEN.

With so many vaccines being given, Weave needs to stay educated on current news and add the vaccination questions to the Wellness Form in order to give providers the most information possible.

Currently the Wellness Form is not editable. The questions no longer fit the AR Health Dept guideline for dentists. We want to be able to edit this Wellness Form. It is confusing to patients and staff to have different questions on the Wellness Form vs in-person screening. It has been 10 months in use. I understand you have a team working on this. Please give us a timeline so we can decide whether to delete the Wellness Form from our current processes.

It would be great if the wellness form could download as jpeg file instead of png OR automatically downloaded into eaglesoft smartdocs once we review it.
Also be able to see & create a patient chart notes while in weave mobile app that integrates into eaglesoft..

We would like to be able to edit the wellness screening questions. The guidelines have changed and we would like to be able to ask questions that are relevant to the current situation. Thank you

We would like to be able to edit the Covid wellness form to included a question for our patients regarding whether they have received the vaccine. And eliminate the question regarding their age since we have that information in their charts. Please let us know if and when this will be possible. Thank you

Customizable wellness forms or a create your own form to send link to patients

PLEASE give us the opportunity to modify the wellness questionnaire. Especially the last question about travel. That question is not accurate anymore like it was last year. It makes us look like we don't know what we are doing, like we aren't staying on top of the changing situation. Please allow us to adapt by allowing us to modify the form as we need to while going forward during this pandemic. Thank you!!!

Can we edit the survey form for COVID we use to send to patients to inquire?

We would like to have the email be an option for our patients to enter if they have one. Many of our older patients do not have an e-mail so they are unable to submit their wellness forms and have to call the office. While it is fine for them to do so it is an inconvenience for all involved.

Hello, We are new to Weave and still in the process of setting up everything. I have someone helping me (Adria) customize our email reminders even though we understood them not to be editable so that is great news! What the doctor's would like to know if there is a way that Weave could edit the "Wellness Form" for us. Ontario, Canada is very specific about the questions we have to ask patients and we would really like to utilize this form but it needs to be changed to conform to our province's guidelines?

We are starting to see more patients that have now had the COVID vaccines or actually had COVID. We would like to see the Wellness Form updated. Is this something in the near future we can expect? I know that we are charting whether the patient has had COVID or the vaccine, so it might be helpful to be able to edit this form. We have had a lot of people calling regarding the Wellness form. A lot of them are mentioning how it doesn't state anything about the new Covid vaccine. Since it is a common thing to get the vaccination now, we believe that it should be listed as one of the questions.

Update wellness form. There are many more questions we can ask our patients, such as if they have tested positive for COVID, if so When? etc.

It would be nice to be able to update the wellness forms

It's getting close to a year now since these forms have been an option for offices and pediatric offices are still not able to use these for our patients. Please make this a priority. I would love to be able to use these instead!

Wellness forms need editing capabilities. These forms are not pediatric office friendly.

Please make an option to receive notifications when a patient has completed their wellness form.

We would like to be alerted (push or pop-up) if someone answers positive to any of the questions on the Wellness Screening form.
