Allowing submitted forms to be sorted by appointment dates AND see All forms submitted
It would be nice to be able to sort submitted forms by date of service (upcoming appointments) so that we can choose to only look at a specific date, like on the current date to see if any patient have submitted forms for the day.
Right now on the Forms dashboard (website), we can select the patient but the their next appointment isn't even showing up.Also it would be nice to see all forms submitted on the desk top app rather than the small amount we are currently able to see.

Form writeback for unified offices
We have a unified office and REALLY would love to have Eaglesoft Med history and other forms write back! Currently it's only supported for single office locations. It's diappointing because we were sold on Weave on this being an available option!

Easier Access to have Patients fill Out Forms
We need easier access (and more reliable) to have patients fill out forms. Currently we send all patients links to fill out forms. But if they do not fill out them on their own time, we set up the iPads for the patients. It is quite a pain to go through the iPad since you have to use the web portal and not the provided app. And now a new issue has popped up where the forms are not even syncing to the patients account, and they are nowhere to be found in the "recently submitted" tab. Online Scheduling Forms: Auto Fill Data from Google Chrome or other web browsers needs to be allowed
When patients are using their personal computer or phone:
The information that is saved, like first name, last name, and email address currently has to be typed or copied and pasted manually
This creates inefficiency and confusion for the customer and patients alike.
Everyone loves auto-fill and relies on it to make each process quicker and more efficient.
Please correct software to allow auto-fill from google chrome, other browsers and personal devices.

From what I gather, it sounds like you're interested in having conditional logic for a Rich Text Box. Since Rich Text Boxes are generally used to display information and don't have labels or input fields, I'm unsure how conditional logic would apply in this context.

Fillable Forms to Text for Signature
Treatment consent forms have to be specific, so we wish there was a way to create a form giving specific info, like # of teeth to be treated with what material, then send to pt for signature. We currently use Adobe esign for this but would like the ability to text these forms to pts.

Filter patient forms when selecting from drop down menu desktop app
When sending forms to patient from weave desktop app you have to select form from drop down menu. All forms that we have created show up but we only use about 4 of them on daily basis. Would like an option to filter which form are in drop down menu.

we need auto fill to be disable on patient forms
When patients are getting forms texted to them, they have an option to auto-fill or manually. We need auto-fill to be disabled. Some phones will not let the patient fill out their health history when they choose auto-fill.

Disable HIPAA form for public forms
Due to new clients not appearing in the system for 24 hours, we are sending public forms to collect new client info. We are a veterinary office, so HIPAA compliance is not an issue.
We would like the option to disable the pre-admission form, or customize it in some way. We do not need to know the client's date of birth, among other info, so it looks a little invasive when we ask.
An alternative solution would be to allow sending forms to non-clients the way we can send text messages, with just a phone number.

there should be a way to send forms to new patient in the forms desk top app and forms web portal without using patient recipient lists. we don't care to use the recipient lint or have one due to our EMR is used with a different software. having to copy the link then go to texting type the number then paste the link takes to long. you should be able to use the desk top app forms section and just type the number in there and attach you forms packet and send.

Forms update or create records in Dentrix with changeable fields through website
Currently patient information keyed in from forms use the First Name, Last Name, Birthdate, and phone number to check the Dentrix data base to determine if the patient exists or is a new patient. This will create duplicates in the Dentrix system because Last names change (Marriage), Phone numbers change through the years, therefore an existing patient will not be found in Dentrix, creating a duplicate family file instead of updating the file of an existing patient. Can we please change these fields to a look up of First Name, Birthdate, and Social Security Number? These are pretty static fields…

Handouts w/o responses needed from the patient
FORMS. We would like to send handouts to patients which are only for information purposes and will not need a response from the patient. Could you make it bypass the pass-code entry and eliminate the screen asking to choose between Auto-fill or manually-fill. This confuses the patient and they wont proceed further in order to view the form/handout.

Turn off Pre-Fill feature for forms
It is best to have an option to turn off the pre-fill feature for forms, since many clients who don't review their answers from previous visits, or don't change the pet's info to the correct pet (especially when sent two forms in one packet) and it's VERY important that they review and correctly answer the questions. Some are not changing answers as needed. We would love to be able to turn this feature off.

Auto reminder to complete forms until they have been completed
When trying to get patients, particularly new patients, to complete the necessary FORM, it would be nice to have an auto-message that continues to go out every1-2 days until the patient clicks on the FORM and completes it.

For Dentrix users - Form with an option of selecting ALL for YES/NO question - For example a Medical Update Form
A provision for patients to be able to select multiple YES/NO answers with one click - click "click here to all NO or YES for all questions below"

Lock patient's name on the form when sending it out
Please allow for an option to lock the patient's name in the form so parents cannot change the name by putting their own info accidentally. It creates a lot of confusion when going back to look for the patient.

Combine incoming forms?
Can we not have all the incoming forms on the dashboard combined? We are having to search through three different folders to find patient paperwork (with write backs, without write backs and the inbox which shows older incoming forms). We would prefer to have them all in one folder. We don't need the "needs review", "approved", "rejected" etc. Just one folder :)
