Regarding the Wellness Form, it would be helpful if there were a section that said "if you have answered YES to any of the questions, please explain"

The Wellness Form doesn't require a patient to sign and date it. What is the legality to this? We stopped using it due to this reason.

Unable to upload a file to eagelsoft without taking extra steps. We would like to be able to choose the format that it is downloaded in.

the option on sending the wellness form to some people and not others should be removed. It should be sent 24 hrs before any patient walks in through the door. Please make sure that there is one option, only confirm if there is a wellness link and HAS to go out to ALL patients, not just unconfirmed patients. This has created a mess in out system as our managers chose the tab that says unconfirmed patients.... well confirmed or unconfirmed, everyone has to answer it!!!!!!!

Wouldn't it be nice to see the COVID-19 Wellness Form Responses without having to download the form first? I would prefer a pop-up instead of having to download, then open the file.

Margarita wants the wellness forms to be more customizable. Looking to implement questions such as:

Have you had COVID?
Have you received treatment for COVID?
How long ago were you treated?

Under the wellness form it would be nice to be able to ck off each document we receive so we know that it has been taken care of.

The Wellness forms are a great idea, however they aren't very helpful when they aren't listed by their appointment date. Nor, can we delete them when we are done. So, there is not a good systematic way to view, print and file them away.
It would be helpful if they were listed per appointment date and if there was a delete feature.

I had a patient say it was hard for them to tell that the wellness firm was submitted. Maybe a pop up saying thank you or something that is more emphasized.

If the patient can receive an acknowledgment after they submit the wellness form that would be very helpful. They submit and then call to see if we got it.

Wellness form only has download option. If we want to just look at it or print it out, it is multiple steps (download, open, then print) Instead of download, maybe just option to open it to view it, and then can decide to download it or print it.

Currently, my front desk staff is checking the wellness forms received on Weave and noting "WF" in each patient's appointment details on Dolphin schedule, so the back team can easily identify who needs the questions asked at check-in. It would be amazing if Weave can automatically integrate with Dolphin to make a visual notation that the wellness form was completed (sort of like the confirmation check mark). PLEASE :)

Now that we are using the wellness form it would be nice if WEAVE could save our patient email addresses (in the WEAVE email app) so we could use these email addresses for a marketing campaign. When I "control print" it cuts off the patient full email address.

We need more questions added to the Covid Wellness screening. We want to know if people have been in contact with anyone who has tested positive for Covid19 in the past 14 days, whether they have been tested for Covid19 in the last 14 days, if the test was positive and if they have traveled more than 100 miles from their home in the past 14 days. This would be very helpful and is very important information for us to know! Please and thank you!

Really love Weave for our texting and reminder software. However, we also utilize the "wellness form" and patients have told us that there is no confirmation that the form was submitted and so it's confusing to them. We have had multiple patients submit it more than once because they didn't know if it went thru. I wonder if there is a way to add that to the form. Thanks!
